肺性脑病患者脑循环改变的研究

摘    要:
目的研究肺性脑病患者脑循环的改变情况。方法对60例轻型、中型及重型肺性脑病(各20例)分别于治疗前和治疗后进行血气分析,经脑循环动力检测仪(CVA)测定脑循环最大血流速度(Vmax)、最大血流量(Qmax)及动态阻抗(DR)。结果各型肺性脑病患者治疗前、后PaCO2比较均有显著性差异(P<001);治疗前、后轻型和中型肺性脑病患者的Vmax、Qmax及DR均有显著性差异(P<001),治疗前、治疗后重型肺性脑病患者的各指标无显著性差异(P>005)。结论肺性脑病患者的脑血管对PaCO2升高较为敏感,有一定的调节能力,随着PaCO2逐渐升高,脑血管对其敏感性降低,脑血管调节功能受到损害。

Abstract:
Objective To observe the changes of brain circulation of patients with pulmoanry encephalopathy patients.Methods Before and after treatment blood gas analysis was conducted in 60 cases with mild,moderate,and serious plmonary encephalopothy meanwhile maximum blood stream pace (Vmax),maximum blood stream quality (Qmax),and dynamic impedance(DR) were measured.Results After the treament all patients with pulmonary encephalopathy exhibited significant decrement of PaCO 2 ( P <0 01).Vmax,Qmax,and DR of mild and moderate encephalopathy were improved significantly ( P <0 01) but not in severe patients ( P >05) .Conclusions The brain circulation of patients with pulmoanry encephalopathy is responsive promptly to the changes of PaCO 2 as an automatically regulation mechanism.However the adaptive mechanism is damaged in serious patients.
