摘    要:目的 探讨急性胰腺炎(AP)患者血气及电解质的变化.方法 将90例AP患者分为轻症组(MAP)和重症组(SAP),分析发病48小时内采集的动脉血气及电解质变化.结果 SAP组的PaCO2、PaO2、HCO3-明显低于MAP组(P<0.05),pH和阴离子间隙(AG)两组无显著差异.在酸碱代谢方面,以呼吸性碱中毒或呼吸性碱中毒并代谢性酸中毒最常见,依次为代谢性酸中毒、呼吸性酸中毒并代谢性酸中毒、代谢性碱中毒.电解质紊乱以低血钙为主,尤其是SAP患者更加明显(P<0.05).结论 AP患者低氧血症、酸碱平衡及电解质紊乱可能在ARDS、MODS发生前即已存在,低氧血症可能是一种早期的MODS信号,通过进行动脉血气及电解质的监测,并进行早期干预,对预防MODS的发生,降低病死率有重要的意义.

Abstract:Objective To study the changes of vigour and electrolyte for 90 acute pancreatitis cases (AP); Methods Dividing 90 cases into MAP and SAP to analyze the changes of artery vigour and electrolyte collected within 48 hour from diseases attacking. Results PaCO2,PaO2 and HCO3-of SAP is obviously lower than MAP's (P<0.05),but there isn't any remarkable difference for the clearance between PH and anion(AG). As for the metabolism of acid and alkali, the alkali poisoning of breath and acid poisoning of breath with acid poisoning of metabolism are most common. There respectively are acid poisoning of metabolism, acid poisoning of breath with acid poisoning of metabolism and alkali poisoning of metabolism. The turbulence of electrolyte mainly indicates the hypocalcemia and SAP is very obvious (P<0.05). Conclusions The hypoxemia, acid-base balance and electrolyte disturbance are existed prior to ARDS and MODS. Hypoxemia perhaps is one of signals for the early MODS, so it is vital to defend MODS happening and reduce death rate through the artery vigour and electrolyte inspection and early intervention.
