慢性肾脏病3期合并高尿酸血症的临床分析

目的探讨慢性肾脏病3期高尿酸血症与慢性肾脏病及相关危险因素的关系。方法对178例慢性肾脏病3期患者进行回顾性研究，根据血尿酸水平，将其分为高尿酸组和尿酸正常组，分析蛋白尿、肾功能及相关危险因素的差异。结果178例慢性。肾脏病3期患者中发生高尿酸血症137例（高尿酸组），发生率为76．9％；尿酸正常者41例（尿酸正常组）。女性患者高尿酸发生率为85．6％，较男性患者高尿酸发病率高，差异有统计学意义（P〈0．05）。高尿酸组与尿酸正常组的蛋白尿、肾功能、血压、血脂比较，差异均无统计学意义（P均〉0．05）。结论高尿酸血症在慢性肾脏病3期患者中发生率高，但高尿酸血症在肾功能受损患者中的致病性有待进一步研究。

Clinical analysis of chronic kidney disease in the third stage with hyperuricemia

Objective To explore the relationship of hyperuricemia with chronic kidney disease （CKD） and associated risk factors. Methods A retrospective study of 178 patients with CKD of the third stage was done,and the patients were divided into high uric acid group （n = 137） and uric acid normal level group （n = 41 ） according to the blood uric acid level. The differences in proteinuria, renal function and the associated risk factors were analyzed. Results In 178 patients with CKD of the third stage,hyperuricemia occurred in 137 cases （high uric acid group） with the incidence being 76. 9% ,and 41 cases had normal level of uric acid （uric acid normal level group）. The high uric acid rate in female patients was 85.6%, significantly higher than that in male patients （P〈0. 05）. There was no signifi- cant difference in proteinuria and kidney function,blood pressure,blood lipids between two groups （P 〉0. 05）. Conclusions The incidence of hyperuricemia in CKD of the third stage is high,but the patho- genicity of high uric acid in patients with impaired renal function needs further study.
