ASST在慢性特发性荨麻疹患者自身免疫机制异常评估中的应用价值

目的 探讨自体血清皮肤试验（ASST）在慢性特发性荨麻疹（CIU）患者自身免疫机制异常评估中的应用价值。方法选取2017年2月至2018年1月我科收治的200例CIU患者作为研究对象，均行ASST检测。根据ASST检测结果，将CIU患者分为阴性组（140例）和阳性组（60例）。比较两组的一般资料、伴发疾病和家族病史、特异性自身抗体检测结果及临床症状体征评分。结果 阳性组的病程长于阴性组，差异具有统计学意义（P<0.05）；两组的性别、年龄、发作频率和症状体征评分无明显差异（P>0.05）。两组伴发过敏性鼻炎、慢性感染疾病及存在过敏性疾病家族史、自身免疫相关疾病家族病史的患者占比无明显差异（P>0.05）；阳性组伴发自身免疫相关疾病的患者占比高于阴性组（P<0.05）。两组的幽门螺杆菌抗体阳性率、乙肝表面抗原阳性率无显著差异（P>0.05）；阳性组的甲状腺微粒体抗体阳性率、甲状腺过氧化物酶抗体阳性率均高于阴性组（P<0.05）。阳性组的风团数量、风团持续时间评分高于阴性组（P<0.05）；两组的风团直径、风团外观、风团每日发作次数评分无显著差异（...

Application value of ASST in the evaluation of autoimmune mechanism abnormalities in patients with chronic idiopathic urticaria

Objective To explore the application value of autologous serum skin test(ASST)in the evaluation of autoimmune mechanism abnormalities in patients with chronic idiopathic urticaria(CIU).Methods A total of 200 CIU patients treated in our department from February 2017 to January 2018 were selected as the research objects,all of whom underwent ASST detection.According to the results of ASST detection,the CIU patients were divided into negative group(140 cases)and positive group(60 cases).The general data,concomitant diseases,family history,the detection results specific autoantibodies and clinical symptoms and signs score of the two groups were compared.Results The course of disease in the positive group was longer than that in the negative group(P<0.05);there were no significant differences in gender,age,attack frequency and symptom and signs score between the two groups(P>0.05).There were no significant differences in the proportions of patients with allergic rhinitis,chronic infectious diseases,family history of allergic diseases and autoimmune related diseases between the two groups(P>0.05);the proportion of patients with family history of autoimmune related diseases in the positive group was higher than that in the negative group(P<0.05).There were no significant differences in the positive rate of Helicobacter pylori antibody and HBsAg between the two groups(P>0.05);the positive rate of thyroid microsomal antibody and thyroid peroxidase antibody in the positive group were higher than those in the negative group(P<0.05).The number and duration of wind mass scores in the positive group were higher than those in the negative group(P<0.05);there were no significant differences in diameter,appearance and daily attack times of wind mass scores between the two groups(P>0.05).Conclusion The use of ASST in the diagnosis of CIU patients can effectively evaluate the situation of autoantibodies.The positive result of ASST indicates that CIU patients are more likely to be accompanied by abnormal autoimmune mechanism,and the positive patients have a longer course of disease,the positive rate of thyroid microsomal antibody and thyroid peroxidase antibody will increase,the number of wind masses will increase,and the duration of wind masses may be longer,which provides more accurate ideas for clinical diagnosis and treatment guidance,and is worthy of promotion.
