HIFU联合地诺孕素治疗子宫腺肌病的疗效观察

目的 观察高强度聚焦超声(HIFU)联合地诺孕素治疗子宫腺肌病的临床疗效。方法 选取120例子宫腺肌病患者，根据随机数字表法分为单纯HIFU组、HIFU联合促性腺激素释放激素激动剂(GnRHa)组及HIFU联合地诺孕素组，每组40例。单纯HIFU组采用HIFU治疗，HIFU联合GnRHa组采用HIFU联合GnRHa治疗，HIFU联合地诺孕素组采用HIFU联合地诺孕素治疗。比较三组治疗前子宫体积、血清糖链抗原125(CA125)水平、痛经与月经量评分。结果 三组治疗前子宫体积、CA125水平、痛经与月经量评分比较，差异均无统计学意义(P>0.05)；治疗后1、2、3个月HIFU联合GnRHa组子宫体积分别为(172.5±16.2)、(158.6±14.1)、(141.6±12.1)cm³,HIFU联合地诺孕素组子宫体积分别为(152.4±11.7)、(139.6±12.8)、(120.6±15.6)cm³，均明显小于单纯HIFU组的(198.6±14.1)、(185.3±14.5)、(178.6±15.0)cm³，差...

Effect of HIFU combined with denorgestrel in the treatment of adenomyosis

Objective To observe the clinical efficacy of high intensity focused ultrasound(HIFU)combined with denorgestrel in the treatment of adenomyosis.Methods 120 patients with adenomyosis were selected and divided into HIFU alone group,HIFU combined with gonadotropin-releasing hormone agonist(GnRHa)group and HIFU combined with denorgestrel group according to random number table,with 40 patients in each group.HIFU alone group was treated with HIFU,HIFU combined with GnRHa group was treated with HIFU combined with GnRHa,HIFU combined with denorgestrel group was treated with HIFU combined with denorgestrel.Uterine volume,serum glucose chain antigen 125(CA125)level,dysmenorrhea and menstrual volume scores were compared before treatment in three groups.Results There were no signifificant differences in uterine volume,CA125 level,dysmenorrhea and menstrual volume score among the three groups before treatment(P>0.05).Uterine volumes of HIFU combined with GnRHa at 1,2 and 3 months after treatment were(172.5±16.2),(158.6±14.1),(141.6±12.1)cm3,respectively.Uterine volume of HIFU combined with denorgestrel group was(152.4±11.7),(139.6±12.8),(120.6±15.6),signifificantly lower than that of HIFU alone group(198.6±14.1),(185.3±14.5),(178.6±15.0)cm3.The difference was statistically significant(P<0.05).CA125 levels in HIFU combined with GnRHa group were(60.7±21.8),(57.2±19.3)and(52.6±17.4)U/ml at 1,2 and 3 months after treatment,respectively(58.3±12.7),(55.2±12.8),(50.3±18.1)U/ml in HIFU combined with denorgestrel group were significantly lower than(81.6±26.5),(80.2±23.2),(78.2±21.4)U/ml in HIFU alone group,respectively.The difference was statistically significant(P<0.05).The dysmenorrhea and menstrual volume scores of HIFU combined GnRHa group and HIFU combined denorgestrel group were significantly lower than those of HIFU combined GnRHa group at 1,2 and 3 months after treatment,and the HIFU combined denorgestrel group was signifificantly lower than that of HIFU combined GnRHa group,the differences were statistically signifificant(P<0.05).Conclusion HIFU combined with denorgestrel is effective in the treatment of adenomyosis.
