老年人结肠息肉临床及病理特点观察

目的观察老年人结肠息肉临床及病理特点。方法对我院2008年1月～2010年1月收治的结肠息肉患者临床资料进行回顾性分析,按照年龄分为两组：60岁以上者为老年组,60岁以下者为中青年组。比较两组患者息肉部位、大小与病理类型及癌变率的关系。结果两组左半结肠息肉的患病率均高于右半结肠（P〈0.05）,其中老年组高于中青年组;病理类型以腺瘤性息肉为主,其中以管状腺瘤最多,绒毛状腺瘤癌变率最高（P〈0.05）;体积大的息肉癌变率较高。结论老年人结肠息肉好发于左半结肠,以腺瘤性息肉为主,癌变率与息肉的体积、绒毛成分含量有关。

Clinical and Pathological Features of Colonic Polyp in the Elderly

Objective To observe the clinical and pathological features of colonic polyp in the elderly.Methods The clinical data of the patients with colonic polyp treated in our hospital from Jan.2008 to Jan.2010 were analyzed retrospectively.According to their age,they were divided into two groups： elderly group（≥60 years old） and young and middle-aged group（60 years old）.The relationship of polyp predilection site and size,pathological type and canceration rate was compared between the two groups.Results The morbidity rate of left half colonic polyp was higher than that of right half colonic polyp in the two groups（P0.05）,higher in the elderly group than in young and middle-aged group.Among adenomatous polyps,tubular adenoma was the most.The canceration rate of villous adenoma was the highest（P0.05）,and the canceration rate of big colonic polyp was higher.Conclusion The polyp predilection site of elderly colonic polyp is in the left half colon and adenomatous polyp is the majority.The canceration rate has a correlation with polyp volume and villus content.
