乳腺癌患者外周血CK19基因表达的检测及临床价值分析

摘    要:目的探索外周血CK19基因表达的检测及在乳腺癌诊断中的应用价值。方法应用逆转录结合荧光定量聚合酶链反应(FQ-PCR)检测乳腺癌MDA-MB453细胞株血液标本模型和280例临床标本的CK19表达,同时检测三磷酸甘油醛脱氢酶(GAPDH)表达作为PCR方法的监控指标。结果逆转录结合FQ-PCR检测MDA-MB453细胞株血液标本模型的灵敏度为107个血细胞中含1个乳腺癌细胞的水平。280例外周血临床标本的CK19表达,乳腺癌组(38/120)与非乳腺癌组(20/80)比较,差异无统计学意义(P>0.05);乳腺癌组(38/120)与正常对照组(2/80)比较,差异有统计学意义(P<0.01);乳腺癌淋巴结转移组(21/53)与未见淋巴结转移组(17/67)比较,差异无统计学意义(P>0.05);乳腺癌术后2年以上者,复发组(3/9)与未复发组(6/31)比较,差异也无统计学意义(P>0.05)。GAPDH表达均阳性。结论CK19是上皮细胞性癌细胞的较特异指标,诊断乳腺癌的特异性不强,但可用作病理学检查的补充,或结合其他特异性指标共同诊断乳腺癌微转移和复发,具有一定的价值。

Abstract:Objective To detect CK19 mRNA expression in peripheral blood of patients with breast cancer and to evaluate its clinical value. Methods CK19 mRNA in breast cancer cell lines MDA-MB453 and 280 blood specimens (120 patients with breast cancer, 80 with other cancers and 80 normal controls) were detected by real-time quantitative polymerase chain reaction (FQ-PCR), and the detection of GAPDH mRNA was used as control. Results There were no significant differences in CK19 mRNA expression between patients with breast cancer and patients with other cancers (38/120 vs 20/80, P>0.05); while there were significant differences between patients with breast cancer and normal controls (38/120 vs 2/80, P<0.01). There were no significant differences in CK19 mRNA expression between patients with lymph nodes metastasis and those with no lymph nodes metastasis (21/53 vs 17/67, P>0.05), between patients with recurrence and those with no recurrence (3/9 vs 6/31, P>0.05). The GAPDH gene expression was positive in all specimens. Conclusion CK19 is a special marker of epithelial cell, and detection of CK19 mRNA in peripheral blood might be used as the complemental diagnostic method for breast cancer.
