异丙酚预处理对肝缺血再灌注损伤的保护作用

目的研究异丙酚预处理对肝脏缺血再灌注损伤的影响。方法60例拟行肝叶切除术的肝癌患者,随机均分为三组:对照组(Ⅰ组),异丙酚组(Ⅱ组)和异丙酚预处理组(Ⅲ组)。于麻醉前、肝门阻断前、肝门阻断15min、肝门开放10min和45min等时点抽血测AST、ALT、SOD活性及MDA的含量。结果①三组AST、ALT在肝脏阻断后15min及肝门开放后10min和45min均见逐渐增高,与麻醉前对比有显著性差异(p<0.05),肝门开放后15min及45min组间对比有显著性差异。②肝门开放后15min及45min,SOD、MDA与麻醉前对比有显著性差异(p<0.05～0.01),组间对比有统计学意义(p<0.05)。结论异丙酚对肝脏缺血再灌注损伤有保护作用,且以预处理途径效果更佳。

STUDY OF PROPOFOL PRETREATMENT PROTECTIVE FUNCTION TO HEPATIC ISCHEMIA-REPERFUSION

Objective To assess the effect of propofol pretreatment on hepatic ischemia-reperfusion(I/R). Methods Sixty hepatoma patients for hepatolobectomy were divided into three groups:control group(I),propofol group(II) and propofol pretreatment group(III).Blood samples were taken to analysis AST, ALT,SOD and MDA concentration before anesthesia,before porta hepatic blockage,15 min after porta hepatic blockage and 10 min、45 min after porta hepatic open.Results ①The concentration of AST and ALT were increased 15 min after porta hepatic blockage and 10 min、45 min after porta hepatic open in all three groups, which were significantly higher than anesthesia before.The concentrations of AST and ALT at 10min and 45min after porta hepatic open also show significant disparity in these three groups.②10 min and 45 min after porta hepatic open,compared with anesthesia before, the concentrations of SOD and MDA were significant disparity(p<0.05~0.01),there were statistical significance among the three groups(p<0.05). Conclusion Propofol has protective effect in live I/R,and the effect of pretreatment is better than other methods.
