肝硬化患者肝功能Child-Push分级与乏力评分的相关性分析

目的通过Chalder乏力评分量表研究乏力与肝硬化Child-Push分级的关系。方法选择肝硬化患者58例,慢性肝病无肝硬化患者15例,健康体检者12例作为研究对象,对肝硬化患者进行Child-Push分级,对所有患者进行Chalder乏力评分量表自评,并分析乏力与肝功能Child分级的相关性。结果 Chalder乏力评分与肝病程度有明显相关性,疲倦和需要更多的时间休息与肝病程度呈正相关,感到精力充沛和感到手脚都很有力量和与肝病呈负相关。Chalder乏力评分与肝硬化Child分级有明显相关性,感到疲倦、需要更多时间休息、虚弱、感到着手做事情有困难的分值随着Child分级的升高而显示出升高的趋势,感到精力充沛和手脚很有力量的分值则呈现下降的趋势。结论乏力在肝硬化和慢性肝病患者中的临床表现可以不尽相同,这些症状的评分与健康人群存在明显差异,其中部分条目与肝硬化Child分级的相关性有统计学意义。

Correlative Analysis between Liver Function Child-Push Grade and Fatigue Scale for the Patients with Cirrhosis

Objective To study the relationship between fatigue and cirrhosis Child-Push grade by Chalder fatigue scale.Methods 58 cases of cirrhosis,15 cases of chronic liver disease without cirrhosis and 12 cases in healthy physical examination were selected as the subjects.Child-push grading was done for the patients with cirrhosis.Afterwards,the self-assessment with Chalder fatigue scale was conducted for all the patients.The correlation between fatigue and liver function Child grade was analyzed.Results Chalder fatigue score was apparently correlative with the severity of liver disease.In the assessment scale,the scores of the items,feeling tired and needing to rest more,were positively correlated with the severity of liver disease.The scores of the items,feeling energetic and feeling strong in the hands and feet were negatively correlated with liver disease.Chalder fatigue score and cirrhosis Child grade were correlative remarkably.In terms of feeling tired,needing to rest more,feeling weak and having trouble starting things,the scores increased in tendency with the increase of Child grade.In terms of feeling energetic and feeling strong in the hands and feet,the scores were down-regulated in tendency.Conclusion Fatigue is manifested differently in clinic for the patients with cirrhosis and chronic liver disease.The scores of these symptoms are remarkably different from those in healthy group,in which,some items are correlative significantly with cirrhosis Child grade in statistics.
