洛阳地区20年间食管病变的变化分析

目的探讨洛阳地区食管癌、食管黏膜上皮不典型增生和食管炎三类食管病变的发病现状及发生规律。方法对20年间经胃镜检查并经病理确诊的10 646例三类食管病变,依据其类别、年龄、性别、城乡差异进行前、后十年对比以及性别年龄段分布,分析其发病过程及流行病学现状。结果后十年与前十年对比,三类病变绝对数值分别增加1258例、297例和279例;发病年龄分别延迟4.05年、4.95年和3.24年;性别构成为男性多于女性,与前十年没有明显变化（P〉0.05）;城乡构成为农村多于城市,食管炎和食管癌城乡构成较前十年没有明显变化（P〉0.05）,但食管黏膜上皮不典型增生农村发病比例较前十年显著增多（P〈0.05）;食管癌和食管黏膜上皮不典型增生男女发病的高峰年龄在60～69岁,而食管炎男女发病的高峰年龄在50～59岁。结论提示食管癌仍是洛阳地区最常见的食管疾病,而食管癌的发生是一个渐进的过程,其发生规律与食管炎和食管黏膜上皮不典型增生密切相关。
