Relationship between radiation lung injury and volume of low dose area in patients with breast cancer after intensity-modulated radiation therapy

Objective To investigate the relationship between the incidence of radiation lung injury and the volume of irradiated low dose area in patients with breast cancer after intensity-modulated radiation therapy(IMRT).Methods 122 patients with breast cancer who received IMRT treatment in our hospital were selected as the research objects.According to the incidence of radiation lung injury,they were divided into the radiation lung injury group and the non radiation lung injury group.The dosimetric factors of radiation lung injury were analyzed by univariate and multivariate Logistic regression analysis.Results Among 122 patients,16 cases had radiation-induced lung injury,the incidence rate was 13.11%.There were statistically significant differences in the percentage of total lung volume(V5),V10,V15,V20,V25,V30,V35,V40,V45 between patients with radiation-induced lung injury and patients without radiation-induced lung injury(P<0.05),V5 and V20 are independent risk factors of radiation-induced lung injury.The receiver operating characteristic curve(ROC)analysis showed that the sensitivity and specificity of V5<59.86%were 88.6%and 91.5%respectively,and the AUC was 0.889;the sensitivity and specificity of V20<38.63%were 71.5%and 83.6%,respectively,and the AUC was 0.788.Conclusion The incidence of radiation lung injury in patients with breast cancer after IMRT treatment is related to the large area of low dose radiation area.Besides V20,V5 can also be an independent predictor of radiation lung injury after IMRT treatment.According to its formulation and adjustment of radiotherapy plan,it is of great significance to reduce the risk of radiation lung injury.
