抗结核药物导致肝功能损害的影响因素分析

目的：分析抗结核药物所致肝功能损害的特点及危险因素,为预防提供依据。方法：收集本院2005年1月-2012年12期间登记的肺结核患者401例,其中发生肝功能损害者64例,分析临床特点并对相关因素如年龄、性别、饮酒、肺结核病类别、乙肝表面抗原携带者进行统计学分析。结果：401例患者中,肝功能损害发生率为15.96%（64/401）,统计显示：HBsAg（＋）、每周饮酒≥500 g、复治、≥60岁的高龄者在肝功能损害中差异有统计学意义（P〈0.05）,其肝功损害的发生率分别是41.07%（23/56）,25.68%（19/74）,19.67%（12/61）,26.97%（48/178）;而性别与药物性肝损害无显著性关联（P〉0.05）。结论：HBsAg（＋）、饮酒、复治及高龄的结核病患者是发生肝功能损害的高危人群,在临床中应引起重视。

Influencing Factors Analysis of Liver Damage Caused by Anti-TB Drugs

Objective：To analyze the characteristics and risk factors of hepatic lesion caused by anti-tuberculosis drugs,and to provide evidence for prevention.Method：401 patients with pulmonary tuberculosis registration collected in our hospital from 2005 to 2012,64 cases of liver damage.Clinical features and analysis of related factors such as age,sex,alcohol,TB category,hepatitis B surface antigen carriers were statistically analyzed.Result：In 401 cases,15.96%（64/401）of patients were liver damage,statistics show：HBsAg（＋）,weekly drinking ≥ 500 g,retreatment,≥ 60years old in the liver function damage were statistically significant（P〈0.05）,the liver function damage rate were respectively 41.07%（23/56）,25.68%（19/74）,19.67%（12/61）,26.97%（48/178）.Sex and drug-induced liver had no significant association（P〈0.05）.Conclusion：HBsAg（＋）,alcohol consumption,the retreatment and old TB patients occurr hepatic dysfunction high-risk groups should be in clinical attention.
