CPT-11与5-Fu/LV方案治疗胃癌术后转移的临床价值

目的观察和评价伊立替康（CPT-11）联合氟尿嘧啶（5-Fu）与亚叶酸钙（LV）双周方案（FOLFIRI）治疗胃癌术后转移的临床疗效和不良反应情况。方法对62例胃癌术后转移患者采用FOLFIRI双周方案治疗,剂量为CPT-11180mg/m2,静脉滴入90min,d1;LV200mg/m2,静脉滴入,d1,d2;5-Fu400mg/m2,静脉推注,d1,d2;5-Fu600mg/m2,持续静脉滴入22h,d1,d2;14d为1个周期。结果 59例患者可评价疗效,其中CR2例,PR19例,SD26例,PD12例,有效率35.6%。不良反应主要为中性粒细胞减少、胆碱能综合征及延迟性腹泻。结论对于胃癌术后转移患者,FOLFIRI双周方案疗效肯定,不良反应可耐受,有重要的临床应用价值。

Clinical value of CPT-11 plus 5-Fu/LV in the treatment of metastatic gastric cancer after operation

Objective To evaluate the efficacy and side-effects of irinotecan（CPT-11）plus fluorouracil/leucovorin（FOLFIRI）in the treatment of metastatic gastric cancer after operation.Methods Sixty-two patients with metastatic gastric cancer after operation were treated with irinotecan 180 mg/m2,ivgtt d1;LV 200 mg/m2ivgtt d1,d2;5-Fu 400 mg/m2,bolus d1,d2,and 600 mg/m2,civ,22 h,d1,d2;two weeks per cycle.Results Of 59 evaluable patients,two showed complete response,19 partial response,26 stable disease and 12 had progress of disease.The response rate was 35.6%.The side effects were mainly neutropenia,cholinergic syndrome and delayed diarrhea.Conclusions CPT-11/5-Fu/LV（FOLFIRI）regimen is an effective and well tolerated treatment for metastatic gastric cancer after operation and has promising value of clinical application.
