多种方法联合互补治疗无法切除的原发性肝癌

目的探讨无法切除的原发性肝癌的治疗方法。方法回顾性总结分析271例无法切除的原发性肝癌患者的临床资料、治疗方法及随访资料。结果 271例无法切除的原发性肝癌患者,根据患者个体病情,联合互补选用下述两种或两种以上的非手术治疗方法 ,包括经导管肝动脉化疗栓塞术、肝癌冷冻治疗、肝癌射频治疗、肝癌无水酒精注射治疗、肝癌放射治疗及免疫治疗。253例患者得到随访,随访率93.4%;42例患者病变范围缩小或局限,全身状况改善,获得二期手术探查机会。其中,34例行肝切除手术,8例行肝癌冷冻手术。全组患者1,3,5年生存率分别为71.0%、37.2%和16.7%。结论无法切除的原发性肝癌,应根据患者个体病情,采用多种方法互补应用实施治疗;一旦治疗后肝癌有手术切除指征,应尽早手术探查,争取手术切除,以求根治。

Comprehensive and complementary therapy for unresectable primary liver cancer

Objective To investigate the therapeutic measures of unresectable primary liver cancer（PLC）.Method The clinical data,treatment measures and followed-up data of 271 patients with unresectable primary liver cancer were analyzed retrospectively.Results For these 271 patients,two or more therapeutic measures were chosen from transcatherter arterial chemoembolization,cryotherapy,radiofrequency ablation（RFA）,percutaneous ethanol injection therapy,radiotherapy to immunotherapy according to their state of the lesions.253 of 271 patients were followed up（93.4%）.Forty-two cases accepted the two-stage operation as the lesions were localized or reduced.Of these 42 patients,34 accepted the hepatectomy,8 accepted the cryotherapy.The overall 1,3 and 5 year survival rates were 71.0%,32.7% and 16.7% respectively.Conclusions Unresectable primary liver cancer should be treated comprehensivly according to the state of the lesions.Surgical treatment should be taken once the lesions can be resected.
