GDF-15和NT-proBNP与肥厚型梗阻性心肌病心功能及心脏磁共振指标的相关性分析

摘    要:目的评价生长分化因子-15(GDF-15)和N末端P型利钠肽原(NT-proBNP)与肥厚型梗阻性心肌病(HOCM)纽约心脏病协会(NYHA)心功能分级及心脏磁共振(MRI)指标的相关性。方法连续入选在阜外医院住院的HOCM患者29例,测定所有患者的GDF-15和NT-proBNP的血浆浓度;同时进行MRI检查。结果在HOCM中,发现Log(GDF-15)与NYHA心功能分级呈显著正相关(r=0.578,P<0.001),而NT-proBNP与纽约心脏病协会NYHA心功能分级未见相关性(r=0.126,P=0.514)。单变量分析和多元逐步回归分析,Log(GDF-15)与NT-proBNP与MRI所测得的左房内径,左室舒张末期内径,室间隔厚度,左室后壁厚度及左室流出道压差均未见显著相关性(P均>0.05)。Log(GDF-15)与Log(NT-proBNP)亦不具有相关性(r=-0.082,P=0.671)。结论 GDF-15可反映HOCM患者心功能的严重程度,但NT-proBNP并不能监测HOCM心功能的进展程度。两者对于HOCM者MRI指标的相关性价值有限。
