摘    要:
目的:评价控制性减压术防治老年重型颅脑损伤并发脑梗死的临床疗效。方法选取2010年3月~2013年6月本院收治的老年重型颅脑损伤患者80例,随机分为两组,其中观察组41例,选用控制性减压术治疗,对照组39例,选用标准大骨瓣减压术治疗,观察并比较两组患者的手术效果、脑梗死发生率、6个月后的恢复情况以及格拉斯哥预后评分(GOS评分)。结果观察组的手术效果与脑梗死情况明显优于对照组(P<0.05);观察组6个月后的GOS评分明显高于对照组(P<0.05)。结论控制性减压术防治老年重型颅脑损伤并发脑梗死的手术效果好,能有效降低脑梗死发生率,术后恢复较好,值得临床推广。

Abstract:
Objective To evaluate the clinical efficacy of controlling decompression in prevention and treatment of el-derly severe craniocerebral injury complicating with cerebral infarction. Methods 80 elderly patients with severe cran-iocerebral injury admitted into our hospital from March 2010 to June 2013 were randomly divided into two groups.In the observation group (n=41),patients were treated by controlling decompression,while in control group (n=39),standard large trauma craniotomy was adopted.The surgical effect,incidence rate of cerebral infarction,recovery after 6 months, and glasgow outcome scale (GOS) score in both groups were observed and compared. Results The surgical effect and occurrence of cerebral infarction in the observation group were obviously superior to those in the control group (P〈0.05). The score of GOS after 6 months surgery in the observation group was higher than that in the control group (P〈0.05). Conclusion Controlling decompression in prevention and treatment of elderly severe craniocerebral injury complicating with cerebral infarction can obtain a satisfactory surgical effect, effectively reduce incidence rate of cerebral infarction, and achieve a good postsurgical recovery,and it is worthy of clinical expansion.
