脊髓亚急性联合变性15例临床分析

目的 :描述脊髓亚急性联合变性的临床特点。方法 :本文报告了 15例脊髓亚急性联合变性 ,并对本病临床表现、实验室检查、常见病因、治疗方法及研究进展作了分析。结果 :15例均有后索、侧索及周围神经损害 ,以周围神经损害为主 10例 ,后索、侧索损害为主 5例 ,精神症状 2例 ,血清VitB12 降低 8例 ,正常 7例。结论 :脊髓亚急性联合变性是由于VitB12 缺乏以脊髓后索、侧索及周围神经损害为主的疾病 ,血清VitB12 降低有助于脊髓亚急性联合变性的诊断 ,但血清VitB12 水平对脊髓亚急性联合变性的诊断意义有待进一步探讨。

Clinical analysis of 15 cases of subacute combined degeneration of spinal cord

Objective:To describe the clinical features of subacute combined degeneration(SCD) of the spinal cord.Methods:15 cases of SCD were reported;the clinical features,laboratory tests,common cause,therapy,and the progress in the study of SCD were also analyzed.Results:All patients had lesions in dorsal columns,lateral columns and peripheral nerves,10 of them had main lesions in peripheral nerves and the rest had main lesions in dorsal and lateral columns.Two patients with mental symptom.8 patients with low serum vitamin B 12 level,and 7 patients were normal.Conclusion:SCD is a disease caused by vitamin B 12 deficiency and main lesions are located in dorsal,lateral columns and peripheral nerves.The low serum vitamin B 12 level helps to diagnose SCD.But the diagnostic significance of a serum vitamin B 12 level to SCD should further be studied.
