社区高血压综合防治效果的观察

目的：了解社区高血压综合防治时，国产或进口产的钙拮抗剂硝苯地平控释片降压药物的治疗效果。方法：采用医患双重问卷追踪观察国产钙拮抗剂硝苯地平控释片欣然或德国产钙拮抗剂硝苯地平控释片拜新同单药／联合其他1线降压药物治疗3月的血压控制情况及社区高血压综合防治的效果。结果：医患双方了解心血管危险分层及血压控制情况的一致率达96．7％。德国产硝苯地平控释片拜新同治疗组，平均血压160／90．7mmHg，治疗1月后检测，平均血压下降31／16．7mmHg，其中单药治疗占53．33％。国产硝苯地平控释片欣然治疗组平均血压159／91mmHg，治疗1月后检测，平均血压下降30／17mmHg，其中单药治疗占46．67％。组间降压效果差异无统计学意义（P〉0．05）。两组治疗3月后血压均值基本稳定在治疗1月后血压的水平。结论：短期内国产或进口钙拮抗剂硝苯地平控释片单药或联合其他降压药的降压效果无差异，但国产制剂治疗成本低可提高患者主动参与防治的依从性。

THE OBSERVATION ON EFFECT OF COMPREHENSIVE PREVENTION AND TREATMENT OF HYPERTENSION IN COMMUNITY.

Objective:To understand the efficiency of antagonist of Ca of nifedipine(slow released) produced by local or foreign producer when applied in comprehensive prevention and treatment of hypertension in community.Method:The questionnaire survey on both side of doctor and patient was adopted to observe the effect of blood pressure control,the comprehensive prevention and treatment that single dose of Xinran(local produced nifedipine)and Baixintong(nifedipine produced in Germany)or conmbined with other first line drug of hypertension in community. Result:Among the doctor and patients who were surveyed,96.7% know about the stratification of cardiovascular risks and the status of blood pressure control.Among the group treated by German nifedipine, the average blood pressure was 160/90.7 mmHg,, it decreased by 31/16.7 mmHg one month later after treatment. And the decrease caused by single dose treatment accounted for 46.67%. Among the group treated by local nifedipine, the average blood pressure was 159/91mmHg , it decreased by 30/17mmHg, and the decrease caused by single dose treatment accounted for 46.67%. The difference between two groups was not significant (P>0.05).The average blood pressure of both groups3 month later was kept at the level of one month later after treatment.Conclusion:The efficiency of antagonist of Ca of nifedipine produced by local and foreign producer to decrease blood pressure was similar in a short period, but the local nifedipine has the lower cost, which can improve the compliancy of patients to receive the treatment.
