眼底动脉硬化的滑车上动脉彩色多普勒超声研究

摘    要:目的 通过对位于眼眶内上缘的眼动脉终末支 -滑车上动脉 (STCA)的彩色多普勒超声 (CDFI)检测 ,分析眼底动脉硬化(OFA)患者的血流动力学改变。方法 对正常对照组 37例和 4 4例 OFA患者眼动脉的出颅段 STCA行 CDFI及脉冲多普勒超声检测。结果 两组 STCA彩色血流信号检出率为 10 0 %。对照组频谱特征与眼动脉相似 ,为正向三峰型 :SP1峰、 SP2峰、 DP峰 ,SP1>SP2 ;其最高血流速度为 38.11cm/ s,与眼动脉相似。OFA患者 STCA的 SP2增高 ,部分 SP2 >SP1,部分 SP1与 SP2融合 ;其收缩期最高血流速度、舒张末期最低血流速度略低于对照组 ,两者比较无显著性差异 ,SP2、搏动指数、阻力指数均高于对照组 ,两组比较有高度显著性差异 (P<0 .0 0 1)。结论  OFA患者 STCA的血流动力学特点符合经颅多普勒 (TCD)脑动脉硬化诊断标准 ,证明 STCA的 CDFI检测是一种方法简便、易于掌握、结果准确、无损伤性的诊断脑动脉硬化的新方法 ,并将脑动脉硬化的血流动力学检测方法 ,由颅前段的颈动脉检测、颅内段的 TCD检测 ,发展到颅后段的 STCA检测。

Abstract:Objective To analyse the hemodynamic changes of ocular fundus arteriosclerosis(OFA)via detecting supratrochlear artery (STCA) which was the end branch of ophthalmic artery at supraorbital foramen.Methods In 37 normal controls and 44 OFA patients, the extracranial portion of STCA was detected by color and pulsed Doppler ultrasound.Results STCA could be displayed at supraorbital foramen with a rate of 100% in the two groups.The frequency spectra of STCA in the control group were similar to that of the ophthalmic artery with three positive waves of SP1,SP2,DP and SP1>SP2.Vs was 38.11 cm/s similar to that of the ophthalmic artery.In patients with OFA,the spectra of STCA showed SP2 was higher or SP2>SP1 or fusion of SP1 and SP2 in some of patients,and Vs and Vd were slightly lower than those of the control group,but with no marked difference,SP2,PI and RI were higher than those in the control group,P<0.001.Conclusions The hemodynamic characteristics of STCA in OFA were coincident with the diagnostic standard of cerebral arteriosclerosis on TCD.Therefore CDFI is a new real-time, accurate noninvasive,convenient and cheap method for diagnosing carotid artery stenosis (CAS).Besides extracranial vascular ultrasound and intracranial TCD,CAS can be detected by the spectrum and hemodynamics of the end branch of ophthalmic artery-STCA.
