2种人工流产术的临床观察

目的 探讨套装式一次性宫腔组织吸引器、盐酸丁卡因胶浆及萘普生栓联合用于人工流产术的临床效果。方法对自愿终止妊娠的早期妊娠妇女158例,按自愿分为2组：观察组（77例）于术前20min由医务人员将萘普生栓0.4g塞入肛门内,术中将盐酸丁卡因胶浆挤在宫颈口上,2min左右宫颈松弛,用套装式一次性宫腔组织吸引器进行吸宫手术。对照组（81例）不采用任何药物,采用传统金属吸引管进行吸宫手术。分别观察受术者宫颈扩张、疼痛、人工流产综合征、手术时间和术中出血量等情况。结果宫颈扩张情况：观察组扩宫显效率为64.9%,对照组为4.9%,P〈0.0001。镇痛效果比较：观察组有效镇痛率81.8%,对照组42%,P〈0.0001。人工流产综合征发生情况比较：观察组人工流产综合征发生率48.1%,对照组为77.8%,P〉0.05。手术时间和术中出血量的比较：观察组手术时间（6.2±1.2）min和出血量（12.8±11.4）mL均少于对照组手术时间（8.2±1.3min）和出血量（22.0±16.0）mL,P〈0.0001。结论套装式一次性宫腔组织吸引器、盐酸丁卡因胶浆及萘普生栓联合用于人工流产术利于宫颈扩张、手术时间短、减少术中出血、镇痛效果好,可以用于10周以内的早期妊娠,是一种安全、经济、简便、镇痛有效而又适用基层的人工流产方法 。

Objective Explore the suit-type organizations to attract a one-time intrauterine device,Tetracaine Hydrochloride Mucilage and naproxen suppository for the clinical effect of induced abortion.Methods The voluntary termination of pregnancy in 158 cases of women in early pregnancy,according to a voluntary divided into two groups：the observation group（77cases） was 20 minutes before surgery by the medical staff will naproxen 0.4g squeezed anal suppository,the surgery will be tetracaine hydrochloride Mucilage by squeezing the mouth of the cervix,2 minutes cervical relaxation,with suit-type disposable intrauterine surgical aspirator to suck Palace,The control group（81 cases） does not use any drugs,use of traditional metal suction tube to suck Palace surgery.Patients who were observed by cervical dilatation,pain,abortion syndrome,operative time and blood loss,and so on.Results Cervical dilatation：The Palace of the observation group expanded markedly effective rate was 64.9%,control group was 4.9%,P0.0001.Comparison of analgesic effect：the observation group rate of 81.8% effective analgesic in the control group 42%,P0.0001.Comparison of the incidence of induced abortion syndrome：the observation group the incidence of induced abortion syndrome,48.1%,control group 77.8%,P0.05.Operative time and blood loss comparison：To observe the operative time（6.2±1.2）min and bleeding volume（12.8±11.4）mL were less than the control group,operative time（8.2±1.3）min and bleeding volume（22.0±16.0）mL,P0.0001.Conclusion Set Disposable uterine aspirator,Tetracaine Hydrochloride Mucilage and naproxen suppository for cervical dilatation in induced abortion help,surgery time is short,less blood loss,analgesic effect is good,can be used within 10 weeks early pregnancy,is a safe,economical,simple,effective and pain of abortion as a method applicable to the grassroots level.
