脑卒中危险因素的病例对照研究

[目的 ]　进一步探索脑卒中的危险因素 ,特别是可干预的危险因素 ,为积极预防和控制脑卒中的发生提供依据。　 [方法 ]　根据上海市 4家地段医院脑卒中报病新发病例 ,按年龄、性别和相同居委配对要求 1∶1选取对照 ,开展病例对照研究 ,采用条件Logistic回归模型进行统计分析。　[结果 ]　高血压史、常吃咸食、经常性紧张、性格急躁四项危险因素进入了多因素条件Logistic回归模型 ,其OR值分别为 7.73( 95 %CI 4.78～ 12 .49)、3.0 4( 95 %CI 1.6 3～ 5 .6 8)、2 .74( 95 %CI 1.5 4～ 4.89)和 2 .92 ( 95 %CI 1.2 7～ 6 .74)。　 [结论 ]　对于脑血管疾病的防治需采取综合干预措施 ,包括控制高血压 ,改变不良的生活方式和饮食习惯 ,以及注意劳逸结合 ,保持良好的精神状态。

A case-control study of risk factors for stroke

To further explore the risk factors for stroke,especially the changeable ones,in order to prevent and control the occurrence of stroke. A 1∶1 matched case-control study was used and the conditional logistic regression model was performed in this study.The cases were those new stroke patients registered in four community hospitals in Shanghai,the controls were selected by the same gender and neighborhood,and each pair's ages was permitted to differ within 5 years. Hypertension,more salt intake,stress and precipitancy entered the multiple conditional logistic regression model,and their ORs were 7.73(95%CI 4.78～12.49),3.04(95%CI 1.63～5.68),2.74(95%CI 1.54～4.89) and 2.92(95%CI 1.27～ 6.74 ),respectively. [Conclusion] It's necessary to implement a comprehensive prevention program,such as hypertension control, lifestyle and dietary habit modification, more attentions to between work and relax, and good psychosis preservation.
