多系统萎缩和帕金森病患者交感神经皮肤反应的对比

目的:探讨交感神经皮肤反应在判断多系统萎缩(MSA)和帕金森病(PD)患者自主神经功能障碍中的价值.方法:利用NDI-400型肌电图仪对18例MSA患者、35例PD患者和30位健康人分别进行交感神经皮肤反应(SSR)检测并对比分析.结果:MSA组SSR异常率(83.33% )显著高于PD组(65.71%),病程≤2年者,MSA组SSR异常率78.57%, PD组SSR异常率为50%,无自主神经症状者,MSA组SSR异常率为33.33%,PD组SSR异常率为30.77%.结论:(1)SSR可定量评价MSA和PD的交感神经功能状态,对两患者组的亚临床自主神经功能障碍有早期诊断价值;(2)MSA引起双侧SSR抑制,PD也可致双侧SSR抑制,但以肢体受累严重侧SSR抑制显著;(3)MSA所致自主神经功能障碍更严重、更广泛.

A Comparative Study on Sympathetic Skin Response in Patients with Multiple System Atrophy and Patients with Parkinson Disease

Objective: To evaluate the significance of sympathetic skin response(SSR) in distinguishing the autonomic never disorder in multiple system atrophy(MSA) from the symptoms of Parkinson disease(PD).Methods:Using NDI-400 EMG instrument,the SSR of 18 MSA patients,35 PD patients and 30 healthy people were dectected and data were compared.Results:The abnormality rate of SSR in patients of group MSA(83.33%) was significantly higher than that(65.71%)in group PD.The abnormality rate of SSR in patients with two-year courses was 78.57% in group MSA and 50% in group PD respectively.In patients without autonomic nerve symptoms the abnormality rates of SSR were 33.33% in group MSA,and 30.77% in group PD.Conclusions:1.SSR might be used as an objective index to asses the condition of sympathetic nerve function in patients with MSA and PD quantitatively,and has some diagnostic value to subclinical autonomic nerve dysfunction.2.In group MSA,SSR might be bilaterally suppressed,while in group PD,SSR suppression mainly occur in lower limbs and is un-equal.3.The autonomic nerve dysfunction caused by MSA is more severe and widespreaded than that caused by PD.
