基层医院如何开展胸腔镜手术及手术成本控制对策

目的探讨如何在基层医院开展电视胸腔镜手术及手术成本控制对策。方法利用自制器械对308例患者行电视胸腔镜手术,通过改进技术、制定不同术式的方法控制手术成本。结果 308例患者中34例胸腔镜辅助小切口,4例中转开胸,共10例出现围手术期并发症,发生率为3.42%,无死亡病例。结论通过自制器械,制定合理的学习曲线,加强手术成本控制,手术费用明显降低,使基层不发达地区患者也能共享微创技术带来的福音。

How to develop video-assisted thoracoscopic surgery and surgical cost control measures in basic hospital

Objective To explore how to develop video-assisted thoracoscopic surgery（VATS） and surgical cost control measures in basic hospital. Methods Clinical data of 308 patients undergoing VATS , through improved technology and methods, surgical operation for different cost control measures Results In 308 eases,34 cases of assisted thoracic small incision, 4 cases were converted to thoracotomy, perioperative complications were 10 cases （3.24%）, no deaths. Conclusions The self-made instruments, formulate a reasonable learning curve, control the operation cost in preoperative, surgical costs decreased significantly, to solve the underdeveloped areas patients can also share the gospel with chest micro-traumatic technique .
