Abstract:
Objective To investigate the relationship between serum 25-hydroxyvitamin D and diabetic kidney disease in patients with type 2 diabetes.Methods 300 patients with type 2 diabetes treated in our hospital from June 2018 to June 2019 were selected as reseach subjects.According to UACR,the patients were divided into diabetic renal disease group(UACR≥30 mg/g,n=72)and normal white egg uria group(UACR<30 mg/g,n=228).Diabetic kidney disease group was divided into large albuminuria group(UACR≥300 mg/g,n=62)and microalbuminuria group(30≤UACR<300 mg/g,n=10).Serum 25-(OH)-D,urine microalbumin/creatinine ratio(UACR),24 h concentration of urine N-acetyl-β-D-glucosidase(UNAG)and Cr was calculated and the UNAG/Cr ratio was calculated.The relationship between UACR and UNAG/Cr and 25-(OH)-D was analyzed.Results There were no significant differences in age levels among normal albuminuria group,microalbuminuria group and the large proteinuria group.As the course of the disease prolonged,the level of UACR gradually increased(P<0.05).The BMI and HbA1c in the microproteinuria group and the large proteinuria group were higher than those in the normal albuminuria group(P<0.05).As the UACR level gradually increased,the proportion of patients with hypertension increased(P<0.05).As the UACR level gradually increased,TG and 25-(OH)-D gradually decreased(P<0.05).The eGFR of the large proteinuria group was significantly lower than that of the microproteinuria group and the normal albuminuria group(P<0.05).UACR was related to gender,age,course of disease,hypertension,HbA1c,and TG(P<0.05).UACR,UNAG/Cr had negative correlation with 25-(OH)-D(P<0.05),and eGFR had no correlation with 25-(OH)-D.Conclusion Serum 25-hydroxyvitamin D deficiency is more common in patients with type 2 diabetes,and 25-(OH)-D is related to renal function indicators.25-hydroxyvitamin D deficiency may cause diabetic kidney disease by affecting glomerular and tubular properties risk increased.
