自发性小脑出血62例外科治疗分析

目的回顾性分析62例自发性小脑出血患者的临床资料，探讨自发性小脑出血外科手术指征、手术方式及治疗疗效。方法根据意识障碍、病情演变过程、颅脑CT所见等采取不同的手术治疗。对62例自发性小脑出血患者进行脑室引流术、血肿引流术、后颅窝开颅血肿清除减压术。结果15例行侧脑室钻孔引流术，11例行血肿引流术，22例行后颅窝开颅清除血肿，14例行后颅窝开颅及侧脑室外引流术。其中治愈25例，好转22例，无效6例，死亡9例。结论早期诊断，根据临床特点及颅脑CT见血肿部位与血肿量行个体化外科手术治疗可提高治愈率，降低死亡率，改善预后。治疗效果主要与意识状态和血肿类型等因素有关。

Analysis of surgical operation treatment for spontaneous cerebellar hemorrhage in 62 cases

Objective To investigate the surgical operation indications and patterns and results in spontaneous cerebellar hemorrhage through retrospective analysis in 62 cases. Methods The different surgical operation treatment for spontaneous cerebellar hemorrhage were 62 cases in this series from January 2000 to July 2006. Results 25 cases were recovered,22 cases were improved greatly, 6 cases were ineffective,9 cases were dead. Conclusion Earlier diagnosis and personal treatment results in better prognosis. Different surgical operations were carried out according to clinical characteristics and type and volume of hematoma in CT scan of brain. The effect of surgical operations related closely to consciousness and types of hematoma.
