腹腔镜治疗早期宫颈癌的临床疗效分析

目的探讨腹腔镜广泛子宫切除术和盆腔淋巴结清扫术治疗早期子宫颈癌的可行性和安全性。方法回顾性分析2010-06～2012-03腹腔镜下广泛子宫切除术加盆腔淋巴结切除术治疗子宫颈癌(Ⅰa-Ⅱb期)32例的临床资料。结果手术成功率100%,术中无1例重要脏器损伤发生,无1例中转开腹及死亡发生。腹腔镜平均手术时间(299.7±127.9)min,术中平均出血量(217.3±130.6)ml,术后肠道功能恢复时间为(2.3±0.2)d,术中未发生血管、肠管及膀胱、输尿管等损伤,术后未出现输尿管阴道瘘、膀胱阴道瘘、深静脉栓塞、淋巴囊肿及腹壁切口处转移等。术后远期并发症膀胱功能障碍共7例。结论腹腔镜广泛子宫切除加盆腔淋巴结切除术治疗早期宫颈癌是安全、可行的。

Clinical analysis of laparoscopic radical hysterectomy and pelvic lymphadenectomy for early cervical cancer

Objective To investigate the feasibility and security after laparoscopic radical hysterectomy and pelvic lymph node dissection for cervical cancer.Methods A retrospective analysis for the clinical data of 32 cases ofⅠa-Ⅱb cervical cancer undergoing laparoscopy surgery in Department of Obstetrics and Gynecology in General Hospital of Beijing Military Area from June 2010 to March 2012 were conducted.Results The successful rate of operation was 100%.No important organ injury occurred during the operation, and no case was converted into the open operation.The mean operative time and the blood loss were(299.7±127.9)min and(217.3±130.6)ml,respectively.The time of bowel function recovery was(2.3±0.2)d.No intraoperative complications occurred.Only 7 cases of bladder dysfunction were found as the long-term complications.Conclusion Radical hysterectomy and pelvic lymphadenectomy under laparoscope could be safe and feasible in the treatment of early cervical cancer.
