脑梗死伴H型高血压的临床特征

目的探讨脑梗死伴H型高血压与脑梗死伴单纯高血压患者之间颅内外血管粥样硬化、神经功能缺损评分、疗效及复发的关系。方法将128例脑梗死患者分成H型高血压组和单纯高血压组,对2组患者的神经功能缺损评分、颈部血管彩超和头颅CTA检查、疗效及复发率进行比较。结果 H型高血压组患者的软斑检出率25%,多发斑块检出率44%,颅外血管狭窄率19.2%,颅内血管狭窄发生率40.1%,治疗2周病情好转率66.7%,1a内脑梗死复发率27.7%,而单纯高血压组分别为10%、22%、6%、22%、76%和11.3%,2组比较差异均有统计学意义（P〈0.05）。结论 脑梗死伴H型高血压患者颅内外血管粥样硬化程度更高,预后更差,复发脑梗死的危险性更高。

Clinical feature of cerebral infarction with H-type hypertension

Objective To evaluate the relationship between cerebral infarction with H-type hypertension and cerebral with hypertension in atherosclerotic,Chinese scale of clinical neurological deficit,prognosis and recurrence of cerebral infarction.Methods One hundred and twenty-eight cases of cerebral infarction patients were divided into H-type hypertension group and hypertension group.The Chinese scale of clinical neurological deficit,result of neck vascular ultrasound and computed tomography angiography（CTA）,prognosis and recurrence of cerebral infarction were compared and analyzed.Results In the H-type hypertension group,the incidence rates of soft plaques,multiple plaques,extracranial atherosclerotic stenosis,intracranial atherosclerotic stenosis,recoverable and recurrence of cerebral infarction in one year were 25%,44%,19.2% and 40.1%,66.7%,27.7%,respectively.In the hypertension group,the incidence rates were 10%,22%,6%,22%,76% and 11.3%,respectively.By analysis of statistics,there were significant differences between the H-type hypertension group and the hypertension group for all above indexes（all P0.05）.Conclusion Cerebral infarction with H-type hypertension has higher level of atherosclerosis,poorer prognosis,and higher risk for recurrence of cerebral infarction.
