周剂量紫杉醇同步放疗治疗老年食管癌的临床观察

摘要]目的观察周剂量紫杉醇同步放疗治疗老年食管癌的近期疗效和安全性。方法86例老年食管癌患者随机分为2组，周剂量紫杉醇同步放疗组（同步组）44例，单纯放疗组（单放组）42例。同步组：紫杉醇75mg·m-2，d1,8,15放疗采用常规分割，照射剂量60～66Gy；单放组：仅接受放疗，方法与同步组相同。结果同步组有效率93．2％，高于单放组的66．7％，比较差异有统计学意义（P〈0．05）。2组主要毒副反应为骨髓抑制、神经毒性、放射性食管炎等，其中同步组Ⅱ-Ⅳ度白细胞减少发生率明显高于单放组（P〈0．05）。结论周剂量紫杉醇同步放疗治疗老年食管癌近期疗效好，毒副反应可耐受。

Weekly Paclitaxel plus Concurrent Radiotherapy in the Treatment of the Elderly Patients with Esophageal Cancer

Objective To investigate the short-term efficacy and safety of weekly paclitaxel plus concurrent ra- diotherapy in the treatment of the elderly patients with esophageal cancer. Methods Eighty-six elderly patients with esophageal cancer were randomly divided into the weekly paclitaxel plus concurrent radiotherapy group （44 cases） and the radiotherapy group （42 cases）. The weekly paclitaxel plus concurrent radiotherapy group： paclitaxel 75 -2 mg~ m , d1.8,15 ; the patients of the two groups received the conventional fractionated radiotherapy, the irradiation dose of 60-66 Gy. Results The response rate was 93.2% in the weekly paclitaxel plus concurrent radiotherapy group, and was 66.7% in the radiotherapy alone group, the difference was statistically significant （P 〈 0.05 ）. The main toxicities were bone marrow depression, neurotoxicity and radiation esophagitis, the incidence of leukopenia with grade Ⅲ and Ⅳ of the weekly paclitaxel plus concurrent radiotherapy group was higher than that of the radio- therapy alone group （P 〈 0.05 ）. Conclusion Weekly paclitaxel plus concurrent radiotherapy is effective in the treatment of the elderly patients with esophageal cancer, and the toxicities can be tolerated.
