女性冠心病抑郁患者pro-BNP和hs-CRP的测定及临床意义

目的观察老年女性冠心病抑郁患者血浆脑钠素（pro-BNP）及超敏C-反应蛋白（Hs-CRP）水平的变化。方法把90例CHD抑郁患者分为急性心肌梗死（AMI）组、不稳定型心绞痛（UAP）组和稳定型心绞痛（SAP）组各30例;另外,按照Gensini积分的三分位间距将研究对象分为A组（Gensini积分〈26分）、B组（Gensini积分26～54分）和C组（Gensini积分〉54分）,检测不同类型CHD患者血浆pro-BNP和Hs-CRP的浓度变化。结果 CHD抑郁患者血浆pro-BNP、Hs-CRP水平及其比值明显高于对照组（p〈0.05）;急性心肌梗死患者血浆pro-BNP、Hs-CRP显著高于不稳定型心绞痛及稳定型心绞痛病人,不稳定型心绞痛患者高于稳定型心绞痛患者（p〈0.05）;C组血浆pro-BNP、Hs-CRP水平明显高于B组及A组,B组显著高于A组（p〈0.05）。结论女性CHD抑郁患者pro-BNP、Hs-CRP水平明显升高,且在一定程度上反映了病情、病变的严重程度,可作为CHD危险分层及病变程度评估的参考指标。

Changes of Plasma pro-BNP,hs-CRP in old women accompanied with depression with Coronary Heart Disease

Objective To investigate the changes of brain natriuretic peptide（pro-BNP） and C-reactive protein（HsCRP） in old women accompanied with depression with Coronary Heart Disease.Methods Ninety patients with CHD were divided into three groups：group A（Gensini score26）,group B（Gensini score between 26 ～ 54） and group C（Gensini score54）.The concentrations of plasma pro-BNP and Hs-CRP in patients were measured.Results The concentrations of plasma pro-BNP and Hs-CRP in the group of CHD were higherthan those in the cont rol group,The concentration of plasma pro-BNP and Hs-CRP ratio in the patients with acute myocardial infarction（AMI） were higher than those in the patients with unstable angina pectoris（UAP） and stable angina pectoris（SAP）,and those in the patients with UAP were higher than those in the patient swith SAP（p 〈0.05）.The concent ration of plasma pro-BNP and Hs-CRP ratio in group C were higher than those in group A and group B,and those in group B were higher than those in group A（p 〈0.05）.Conclusion The concent ration of plasma pro-BNP and Hs-CRP ratio in old women accompanied with depression with Coronary Heart Disease are higher than those in the control group,and to a certain extent,they may reflect the severity of the illness and be used as reference indexes for risk stratification and severity evaluation of CHD.
