巨细胞病毒感染与自然流产及免疫细胞因子水平的关联性分析

摘    要:目的:分析探讨人巨细胞病毒(HCMV)感染与自然流产及免疫因子水平的关联性。方法:研究对象为我院收治的稽留流产孕妇89例和同期正常妊娠孕妇63例,采用聚合酶链式反应(PCR)检测两组孕妇外周血中的HCMV-DNA,并进行组间比较,分析HCMV感染与自然流产的关系。根据孕妇HCMV感染的检测结果,将152例孕妇分为感染组与非感染组,采用ELISA检测两组孕妇血清中免疫相关因子IL-1、IL-6和IFN的表达水平,比较两组差异并分析巨细胞病毒感染与免疫反应的关联性。结果:发生自然稽留流产孕妇的HCMV感染阳性率(32.58%)明显高于正常妊娠的孕妇(9.52%),两组间存在明显差异(P<0.05);HCMV感染组患者的血清IFN、IL-1和IL-6水平均明显高于非感染组(P<0.05);针对两组孕妇的血清免疫细胞因子IL-1、IL-6和IFN的表达水平与HCMV感染的Logistic回归分析,结果显示血清IFN、IL-1、IL-6的最大似然估计值和OR值均>0,则说明血清IFN、IL-1、IL-6水平与HCMV感染存在正相关,孕妇血清中的IFN、IL-1、IL-6表达水平越高,孕妇发生HCMV感染的概率越高。结论:孕妇发生HCMV感染可以增大胚胎感染的风险,进而导致自然流产的发生,而IL-1、IL-6和IFN等免疫反应相关因子与HCMV感染的病理过程密切相关,可以作为孕妇HCMV感染的辅助血清检测指标。

Abstract:Objective:To investigate the association between cytomegalovirus(HCMV)infection and spontaneous abortion and immune factors.Methods:Subjects were admitted to our hospital for treatment of abortion 89 cases of pregnant women and 63 pregnant women with normal pregnancy over the same period.The HCMV-DNA in peripheral blood of two groups of pregnant women was detected by polymerase chain reaction(PCR),and the relationship between HCMV infection and spontaneous abortion was analyzed.According to the results of HCMV infection in pregnant women,152 pregnant women were divided into infected group and non-infected group.The serum levels of IL-1,IL-6 and IFN were detected by ELISA.The differences between the two groups were analyzed and the association between cytomegalovirus infection and immune response was analyzed.Results:The positive rate of HCMV infection(32.58%)in pregnant women with spontaneous abortion was significantly higher than that in normal pregnant women(9.52%),and there was significant difference between the two groups(P<0.05).Serum levels of IL-1,IL-6 and IFN in serum of HCMV-infected patients were significantly higher than those of non-infected patients(P<0.05).Logistic regression analysis of serum level of IL-1,IL-6 and IFN in two groups of pregnant women with HCMV infection,the results showed that the maximum likelihood of the serum IFN,IL-1,IL-6 and OR values were>0,the serum IFN,IL-1,IL-6 levels and HCMV infection was positively correlated,pregnant women in the serum IFN,IL-1,IL-6 expression level,the higher the risk of HCMV infection in pregnant women.Conclusion:HCMV infection in pregnant women can increase the risk of embryo infection,which leads to the occurrence of spontaneous abortion.The related factors of immune response such as IL-1,IL-6 and IFN are closely related to the pathological process of HCMV infection and can be used as an auxiliary serum detection index for HCMV infection in pregnant women.
