海南省三亚市2015—2017年副溶血性弧菌血清型分布和耐药性分析

摘    要:目的 通过对三亚市2015年8月—2017年12月来自食物中毒病人标本和海产品中副溶血性弧菌标本进行血清型分布及耐药性比较,为指导临床合理用药提供依据。方法 参照GB/T4789.7-2013方法进行血清分型、K-B琼脂纸片扩散法进行耐药性检测。结果 从食物中毒病人肛拭子中分离出38株副溶血性弧菌,分属4种血清群,分别是O4群23株(60.53%)、O3群11株(28.95%)、O1群3株(7.89%)、O8群1株(2.63%);完全分型32株,分型率达到了84.21%;海产品中分离出副溶血性弧菌60株,分属9种血清群,分别是O1、O2、 O3、 O4、 O5、 O6、 O7 、O10、 O11;完全分型26株,分型率达43.33%。从中选76株作药敏试验,结果表明:对诺氟沙星、头孢曲松、氧氟沙星、庆大霉素、氯霉素、头孢克洛、妥布霉素、环丙沙星、洛美沙星、依诺沙星、亚胺培南、萘啶酸敏感性达到了100%,对青霉素G、苯唑西林、氨苄青霉素耐药性达到了100%。 结论 来自食物中毒病人和海产品中的副溶血性弧菌中分出的血清群型不一致,O4:K8、O3:K6是引起食物中毒的优势血清型,而海产品分离株未见优势血清型,且血清群和血清型呈现多样性,主要是以O2、O1群为主;三亚市副溶血性弧菌没有产生广泛的耐药性变异。

Abstract:Objective The serotype distribution and drug resistance of Vibrio parahaemolyticus samples from food poisoning patients in Sanya City from August 2015 to December 2017 were compared to provides a basis for guiding clinical rational drug use. Methods The drug resistance was tested by serotyping and K-B agar diffusion method according to GB/T4789.7-2013 method. Results For the 38 strains isolated from the anal swabs of food poisoning patients, 4 serogroups were isolated, 23 strains of O4 (60.53%), 11 of O3 (28.95%), 3 of O1 (7.89%) and 1 of O8 (2.63%) were identified. Totally 32 strains (84.21%) were completely serotyped. For 60 strains isolated from offshore seafood, 9 serotypes (O1, O2, O3, O4, O5, O6, O7, O10 and 11) were identified and 26 (43.33%) were completely serotyped. Drug sensitive test was performed for 76 strains from the patients with food poisoning and seafood; the sensitive rate of the strains was 100% to norfloxacin, ceftriaxone, ofloxacin, gentamicin, chloramphenicol, cefaclor, tobramycin, ciprofloxacin, imipenem, and nalidixic acid and the drug resistance rate of the strains was 100% to ampicillin, penicillinG, and benzene azole. Conclusion The serogroups from Vibrio parahaemolyticus in food poisoning patients and offshore fishing products are inconsistent, O4: K8, O3: K6 are the dominant serotypes that cause food poisoning, However, the aquatic product isolates do not have dominant serotypes, and the serogroups show diversity, mainly based on O2 and O1 groups; Vibrio parahaemolyticus in this area do not produce extensive resistance mutations.
