活动翼矫正器治疗双颌前突临床研究

目的：探讨活动翼矫正器治疗双颌前突临床疗效。方法：10例双颌前突病例,男4例,女6例。采用活动翼矫正器,拔除四个第一前磨牙。治疗前后拍摄头颅侧位定位片。测量治疗前后软硬组织侧貌变化指标,并对测量结果进行T检验。结果：SNA,SNB,SN-MP,PP-MP及Y轴治疗前后差异无统计学意义,提示上下颌骨矢状及垂直向无明显变化。U1-PP（mm）,U6-PP（mm）,治疗前后差异无统计学意义,L1-MP（mm）,L6-MP（mm）,U6-Y平面（mm）,L6-Y平面（mm）治疗前后差异有统计学意义。U1-SN（°）,L1-MP（°）,U1-L1（°）,治疗前后差异有统计学意义,提示上下前牙倾斜度和突度明显减小。上唇-E平面（mm）,下唇-E平面（mm）治疗前后差异有统计学意义,提示上下唇突度明显减小。结论：活动翼矫正器拔牙矫正双颌前突病例,矫治后前牙突度及软组织侧貌均有明显改善。

A clinical study on the treatment of bimaxillary protrusion with active-wing appliance

Objective：To study the clinical effect of treating bimaxillary protrusion with active-wing appliance. Method：10 patients with bimaxillary protrusion,including 4 males and 6 females,were selected to have their four first premolars extracted and to be treated with the active-wing appliance invented by Chen Qifeng. Cephalometric radiographs were taken before and after this treatment. The indexes of the facial profiles＇ changes were analysed by T-test. Result：There were no significant differences on SNA,SNB,SN-MP,PP-MP and Y-aixs before and after the treatment,which indicated that the sagital and vertical direction of the maxillary and the mandible did not change significantly. U1-PP（mm）and U6-PP（mm） did not have significant changes while L1-MP （mm）,L6-MP （mm）and U6-Y Plane （mm）did have. The inclination and protrusion of the upper and lower anterior teeth decreased significantly,as U1-SN（°）,L1-MP（°）and U1-L1（°）changed greatly. The upper lip E Plane （mm）and lower lip E Plane （mm）had significant changes,suggesting that the protrusion of upper and lower lips decreased significantly. Conclusion：By using active-wing appliance,bimaxillary protrusion can be treated effectively with a remarkable improvement of the protrusion and facial profiles.
