鼻咽癌的CT诊断和鉴别诊断

目的总结影像学分析经验，提高鼻咽癌的CT诊断水平。方法回顾性研究32例经病理证实的鼻咽癌，着重就鼻咽癌CT影像特点、CT诊断及鉴别诊断进行分析。结果34例患者中，平扫CT片中全部出现咽隐窝变浅或消失，绝大部分病例显示病灶侵犯咽旁间隙及邻近组织结构，12例出现颅底骨质及颅内侵犯；颈动脉鞘区受累22例，颈淋巴结转移15例。增强扫描，病灶出现不同程度强化。结论根据典型的CT表现，一般不难做出正确诊断，鼻咽癌还需与鼻咽部其他肿瘤相鉴别。

CT scanning in the diagnosis and differential diagnosis of nasopharyngeal carcinoma

Objective To discuss the practical use of CT scanning in the diagnosis and differential diagnosis of nasopharyngeal carcinoma(NPC) on the basis of clinical experience summary.Methods A retrospective study was carried out among 32 cases with NPC,diagnosed based on pathohistology in our Hospital. The emphasis was put on its image features and the key points for diagnosis and differential diagnosis of NPC. Results On the plain films of 34 cases, all showed the lacuna pharynges shallow or disappeared, most appeared the lesion involving the lateral pharyngeal space and its adjacent structures,12 had bone involvement of skull base and intracranial invasion,22 were with the lesion involving the carotid sheath, and 15 displayed metastasis to the neck lymph nodes. On the intensive films, various degrees of enhancement could be observed among these cases.Conclusions It is not very difficult to make out a definite diagnosis for NPC based on the findings on the typical image features of CT. However, the more important thing is to do elaborately planed program of differential diagnosis from those tumors possibly occurred in the nasopharynx.
