子宫动脉栓塞术治疗子宫肌瘤合并继发贫血的临床观察

目的评价子宫动脉栓塞术（UAE）治疗子宫肌瘤合并继发贫血的临床疗效。方法29例子宫肌瘤合并继发贫血的患者，采用Seldinger技术行股动脉插管2，超选择插入双侧子宫动脉后，经导管注入栓塞颗粒阻断子宫肌瘤血供。治疗前及治疗后6个月B超分别测量子宫和子宫肌瘤体积以及检测血红蛋白的变化。结果29例患者子宫和子宫肌瘤体积由术前211．32±90．54cm^3和76．91±66．03cm^3缩小车术后142：17±67．09cm^3和40．94±57．32cm^3（P〈0．01）；血红蛋白术前由86．76±17．34g/L恢复至术后113．24±14．02g/L（P〈0．01）。治疗经量增多和经期延长患者的有效率分别为93．10％（27／29）和90％（18／20）。结论UAE能明显改善子宫肌瘤合并继发贫血患者的临床症状，无需输血来纠正贫血，缩小子宫和子宫肌瘤的体积。

Clinical observation of uterine arterial embolization for uterine myoma secondary anemia

Objective： To evaluate the clinical effect of uterine arterial embolization on the treatment of uterine myoma in patients with secondary anemia. Methods： Twenty -nine patients with uterine myoma and secondary anemia underwent superselective embolization of the uterine artery with embolization particles by means of Selding method. Ultrasound was used to measure the size of the uterine and the myoma and the changes of hemoglobin. Results ： The volume of the uterine and the myoma in the 29 patients decreased from 211.32 ±90.54 cm^3 and 76.91 ±66.03 cm^3 to 142.17 ±67.09 cm^3 and 40.94 ±57.32 cm^3, respectively. Hemoglobin increased from 86.76 ± 17.34 g/L to 113.24 ± 14.02 g/L. 27 （93.10%） in the patients with increased menstrual quantity and 18 （ 90% ） in the patients with prolonged menstrual period. Conclusion： UAE is an effective method for the treatment of uterine myoma in patients with secondary anemia.
