轻度创伤性脑损伤的扩散张量成像研究

摘    要:
目的探讨MR扩散张量成像(DTI)对轻度创伤性脑损伤(m TBI)的诊断价值。材料与方法对35例轻度m TBI患者(病例组)和15名健康对照者(对照组)分别行常规MR、DTI检查。选择双侧额叶白质、双侧内囊区、胼胝体膝部及压部中线位置各一个点(共6个)为感兴趣区,分别测定FA值、ADC值。将病例组与对照组各部位的FA值、ADC值进行统计学分析。结果病例组与对照组间各部位ADC值均无显著性差异。双侧额叶白质区、双侧内囊区及胼胝体压部FA值病例组与对照组均有统计学差异(P〈0.05)。结论 FA值能定量测定轻度创伤性脑损伤后脑组织水分子代谢的情况,DTI可以作为一项客观指标,定量评估轻度脑外伤患者的病情,并为对病情发展和预后的观察带来可能。

Abstract:
Objective: To evaluate the value of diffusion tensor imaging(DTI) in mild traumatic brain injury(m TBI). Materials and Methods: Thirty-five cases of m TBI patients(case group) and 15 healthy controls(control group) underwent taditional MRI and DTI scans. The bilateral white matter of the frontal lobes, bilateral anterior region, the genu and splenium of corpus callosum midline position(a total of 6) were chosen as the region of interest. FA value, ADC value of each region of interest were measured. The FA value, ADC value in all parts of the case group and the control group were statistically analyzed. Results: There was no signifi cant difference between the case group and control group in each part for ADC value. The FA value in bilateral frontal lobe white matter, internal capsule and splenium of corpus callosum were statistically significant between the case group and control group(P〈0.05). Conclusions: The value of FA can quantitatively determine the metabolism of water molecules for mild traumatic cerebral. DTI can be used as an objective index, to accurately assess the severity and prognosis of the patients with mild traumatic brain injury.
