音乐疗法配合高速涡轮机微创拔牙术在阻生牙拔除中的临床应用

摘    要:
目的探讨音乐疗法配合高速涡轮机微创拔牙术在阻生牙拔除中的作用。方法选取迁安市人民医院口腔科收治的阻生牙手术患者336例,分为研究组及对照组,每组各168例。研究组给予音乐疗法配合高速涡轮机微创拔牙辅助拔除阻生牙,对照组给予传统凿骨劈冠法拔除阻生牙。观察并比较两组患者的手术时间、疼痛持续时间、术后出血、张口受限程度、干槽症以及第三磨牙和下牙槽神经(IAN)的损伤例数。结果研究组患者的手术时间和疼痛持续时间均明显短于对照组,差异有统计学意义(P〈0.05)。研究组患者张口受限程度以轻度为主,而对照组以中度为主;研究组张口受限重度发生率为2.97%,明显低于对照组的19.64%,差异有统计学意义(P〈0.05)。研究组重度疼痛发生率为6.55%,明显低于对照组的14.29%,差异有统计学意义(P〈0.05)。研究组患者术后出血、干槽症及IAN损伤的发生率分别为1.79%、1.79%、1.19%,明显低于对照组的8.93%、10.12%、6.55%,差异有统计学意义(P〈0.05)。结论采用音乐疗法配合高速涡轮机微创拔牙术拔除阻生牙,能够明显减轻患者的疼痛,缩短手术时间,同时,降低患者术后并发症的发生率,安全省时,值得推广。

Abstract:
Objective To explore the effects of music therapy combined with high speed turbine minimally invasive ex odontia in extraction of impacted tooth.Methods 336 cases of patients underwent impacted tooth operation in Department of Stomatology,the People′s Hospital of Qian'an City were selected,then all patients were divided into the re search group and the control group,each group had 168 cases.Patients in the research group were given music therapy combined with high speed turbine minimally invasive exodontia for extraction of impacted tooth,while,patients in the control group were given conventional carved bone splitting method for extraction of impacted tooth.The operation time,pain duration,postoperative bleeding,limited mouth opening degree,dry chamfer disease,and the third molar and inferior alveolar nerve(IAN) damage cases between the two groups of patients were observed and compared.Results The operation time and pain duration in patients of the research group were shorter than those of the control group,the difference was statistically significant(P 0.05).Limited mouth opening degree of patients in the research group was mainly mild,but which in the control group was mainly moderate;the incidence of severe limitation of mouth opening in the research group was 2.97%,which was significantly lower than the control group(19.64%),the difference was statistically significant(P 0.05).The incidence rate of severe pain in the research group was 6.55%,which was sig nificantly lower than in the control group(14.29%),the difference was statistically significant(P 0.05).The postoperative bleeding,incidence of dry chamfer disease and injury of IAN in the research group were 1.79%,1.79%,1.19% respectively,which were significantly lower than those in the control group(8.93%,10.12%,6.55%),the differences were statistically significant(P 0.05).Conclusion The music therapy combined with high speed turbine minimally invasive exodontias for extraction of impacted tooth can significantly reduce patients' pain,shorten the operation time,at the same time,reduce the incidence of postoperative complications in patients,which is safety to save time.It is worthy of clinical application.
