LGT蛋白质组与肝硬化患者预后的相关性研究

目的探讨肝硬化患者血清中是否存在LGT蛋白质组的表达,初步分析LGT蛋白质组基本临床意义指向。方法应用美国CipherGen公司表面增强激光解析电离飞行时间质谱仪和蛋白芯片获得肝硬化患者血清中的蛋白质指纹图谱,用Biomarker Wizard软件进行分析,以在指纹图上,质荷比(m/z)为11100+H至11900+H之间出现一峰簇样的指纹标志为阳性诊断标准,将LGT蛋白质组动态变化分为4组,观察4组中的病死率。结果肝硬化患者血清中存在LGT蛋白质组。LGT蛋白质组持续阳性组病死率为87%;持续阴性组病死率为6%;由阳性转成阴性组病死率为10%;由阴性转成阳性组病死率为30%。结论肝硬化患者血清中存在LGT蛋白质组表达,持续阳性表达会危及患者的生命;持续阴性表达可以提示患者病情稳定,近期没有恶化和死亡的发生;由阳性表达转阴性提示病情好转;由阴性转阳性提示病情恶化。因此,将其作为一种临床诊断平台,对预测病情发展及预后判断有重要的临床指导意义,为治疗方案的改善提供了新的思路。

Correlation between LGT proteome and liver cirrhosis

Objective To investigate whether LGT （lost goodwill target） proteome express in liver cirrhosis and to analyze the clinical significance of LGT proteome. Methods Protein fingerprints from patients with cirrhosis were obtained with PBS-Ⅱ and protein chip （CipherGen USA） and analysed with Biomarker Wizard. The positive diagnostic criteria was defined as presence of a mass cluster within the range of m/z values between 11100＋H and 11900＋H. The mortality from cirrhosis was compared among 4 groups of patients classified according to the dynamic changes of LGT proteome. Results LGT proteome was found to be present in serum of patients with cirrhosis. The mortality was 87% in patients with persistently positive LGT proteome, 6% in those with persistently negative results, 10% in those with positive to negative conversion and 30% in those with negative to positive conversion. Conclusion The LGT proteome was detected in serum of patients with cirrhosis. While persistent expression of LGT proteome can be life- threatening, persistently negative expression may imply a stable condition. Conversion from positive to negative results may suggest improvement, and that from negative to positive results could mean deteriorated illness. Therefore, as a clinical diagnostic tool, LGT proteome can be used to predict disease progression and prognosis, which pave a new way for refinement of current treatment.
