| 摘 要： | 目的 观察星状神经节(SGB)连续置管术治疗颈源性头痛的效果.方法 选择36例经确诊惠颈源性头痛的患者,将其随机分为SGB连续阻滞配合神经妥乐平组(联合组)与单纯SGB连续阻滞组(阻滞组),每组18例.联合组除给予sGB连续置管带泵外,同时给予神经妥乐平制剂7.2 U,每日1次静滴,连续应用15 d为一疗程.观察治疗后1、7、15、30 d视觉模拟评分法(vAs)评分、睡眠质量(Qs)评分、颈椎活动度(ROM)、麻木和针刺感改善情况、头痛缓解率及镇痛药科洛曲的用量.结果 与治疗前比较,治疗后2组VAS、QS、ROM评分均明显下降,麻木及针刺感均明显改善(P<0.05);治疗后2组比较,VAS、QS、ROM评分、麻木及针刺感改善率的差异均有统计学意义(P<0.05).治疗后2组镇痛药科洛曲用量较治疗前均降低,联合组减量明显,最后停药.结论 连续sGB治疗颈源性头痛疗效显著,不良反应小,患者耐受性和依从性佳,联合神经妥乐平治疗对患者疼痛、麻木、针刺等神经症状的改善优于单纯SGB治疗.
