Atlantoaxial pedicle screw fixation for odontoid fracture associated with atlantoaxial joint dislocation

Objective To explore the clinical curative effect of atlantoaxial pedicle screw fixation for odontoid fractures associated with atlantoaxial joint dislocation.Methods From December 2006 to March 2009,9 patients with an odontoid fracture associated with atlantoaxial joint dislocation underwent atlantoaxial pedicle screw fixation.There were 7 males and 2 femals,with a mean age of 39 years old（ranged from 19 to 56 years old）.Fresh fractures of 7 cases and old fractures of 2 cases.The odontoid fractures were classified according to Anderson-D＇Alonzo classification： 7 cases of type Ⅱand 2 cases of type Ⅲ.Neurologic function were evaluated according to Japanese Orthopaedic Association（JOA） score,the preoperative mean JOA score was 11.2（9.0-15.0）.Results All cases were complete reposition without vertebral artery,spinal cord or never root injuried.The out-of-bed activity was at 3-6 d postoperative.All cases were followed up for 3-20 months,with a mean period of 15 months,clinical symptom were improved visibly.Postoperative 12 months roentgenograph showed all odontoid fractures had healed,with no loosen or broken of the internal fixation.The mean JOA scores was 15.4（13.0-17.0） at 12month postoperative.Conclusion Atlantoaxial pedicle screw fixation for odontoid fracture associated with atlantoaxial joint dislocation was effective and safety.
