脊髓栓系综合征的影像学评价

目的 探讨脊髓栓系综合征的CT、MRI表现以及在该病诊断中的价值.方法 分析经手术病理证实的45例脊髓栓系综合征患者的CT、MRI资料,再与手术病理对照.结果 45例脊髓栓系综合征中,CT诊断脊髓低位39例,漏诊6例.脊柱裂41例,椎体畸形脊柱侧弯7例,脊髓纵裂3例,其中骨性分隔1例,诊断准确率100%.MRI诊断脊髓低位45例,脊柱裂39例,漏诊6例,椎体畸形脊柱侧弯7例,脊髓纵裂3例,对1例骨性分隔显示不清.结论 脊髓栓系综合征伴发复杂骨骼畸形的患者,MRI结合CT诊断,对患者一次性手术能提供更加详细的信息,提高诊断准确性,减少漏诊.

Imaging Evaluation of Tethered Cord Syndrome

Objective To explore the features and diagnosis value of CT/MRI in the diagnosis of tethered cord syndrome. Methods CT/MRI data of 45 cases of tethered cord syndrome verified by surgical pathology were analyzed and then compared with the surgical pathology. Results 39 of the 45 cases were diagnosed as low spinal cord by CT, 6 missed. 41 cases of bifid spine, 7 cases of scoliosis with vertebral deformity, 3 cases of diastematomyelia and 1 case of bony septation were all accurately diagnosed 100% by CT. With MRI, 45 cases were all diagnosed as low spinal cord. 39 cases of bifid spine were diagnosed, 6 missed. 7 cases of scoliosis with vertebral deformity, 3 cases of diastemtomyelia were all diagnosed but one case of bony septation was not clearly displayed. Conclusion For cases of tethered cord syndrome with concomitant complex bone deformity, MRI combined with CT could provide more detailed information for their operations and thus increase the diagnosis accuracy and reduce missed diagnosis.
