中老年髋骨骨折患者血清脂联素和瘦素的水平研究

目的研究血清脂联素和瘦素水平与中老年人髋骨骨折的关系。方法选择2015年1月—12月在延安市人民医院骨科治疗的245例髋骨骨折患者。其中男性130例,女性115例;平均年龄64.26岁。另选择同时期在延安市人民医院体检中心体检的50岁以上健康人群510例作为对照,其中男性263例,女性247例;平均年龄62.20岁。应用放射免疫分析法测定245例髋骨骨折患者和510例健康对照的血清脂联素和瘦素水平,比较两组临床资料数据,包括年龄、BMI、收缩压、舒张压、吸烟、饮酒、锻炼、甘油三脂、低密度脂蛋白胆固醇等。采用Logistic回归分析计算不同脂联素和瘦素水平组发生髋骨骨折的风险。结果病例组年龄、BMI、收缩压、舒张压、甘油三脂、低密度脂蛋白胆固醇、脂联素和瘦素的水平显著高于对照组,差异有统计学意义(P0.05);病例组低密度脂蛋白胆固醇水平显著低于对照组,差异有统计学意义(P0.05)。病例组吸烟以及饮酒人群所占的比例显著高于对照组,差异有统计学意义(P0.05);而病例组进行体育锻炼的人群则显著低于对照组,差异有统计学意义(P0.05)。血清脂联素和瘦素水平越高,髋骨骨折的发生风险越大(P0.05),脂联素和瘦素最高水平组发生骨折的风险分别为最低水平组的1.40倍(OR,1.40;95%CI,1.09~1.77)和1.75倍(OR,1.75;95%CI,1.18~2.31)。结论血清脂联素和瘦素水平是髋骨骨折的独立危险因素。

Relationships between serum adiponectin,leptin and hip fracture in middle-aged and elderly

Objective To investigate the relationships between serum adiponectin ,leptin and hip fractures in middle-aged and elderly . Methods Totally 245 hip fracture patients who received treatment in Yan 'an People's Hos-pital from Jan.to Dec.2015 were recruited in our study,with 130 men and 115 women,and the mean age was (64.26 ±6.33) years.A total of 510 non-facture individuals who took health examination in the physical examination center of Yan 'an People's Hospital during the same period were taken as controls ,with 263 men and 247 women,and the mean age was (62.20 ±7.03) years.Serum adiponection and leptin of the 245 patients and 510 controls were meas-ured by radioimmunoassay.Clinical data of the case group and controls were compared ,including age,BMI,systolic blood pressure (SBP),diastolic blood pressure (DBP),smoking,drinking,physical activity,low density lipoprotein cholesterol ( LDL-C) ,adiponection ,and leptin and so on .Logistic regression models were used to estimate the associa-tion between elevated levels of serum adiponection and leptin and risk of hip fracture .Results The age,BMI,SBP, DBP,triglyceride (TG),LDL-C,diponection and leptin of the case group was significantly higher than those of the controls ( P<0.05) ,while the low density lipoprotein cholesterol ( HDL-C) level of the case group was significantly lower than that of the control group (P<0.05).The proportion of people smoking and drinking in the case group was significantly higher than that of the control group (P<0.05).The proportion of people who do physical activity of the case group was significantly lower than that of the control group (P<0.05).Higher levels of serum adiponection and leptin were related to higher risk of hip fracture .The risk rate of fracture in the group with the highest serum adipone-ction and leptin level was 1.4 times than that in the group with the lowest serum adiponection and leptin level .Con-clusion Serum adiponectin and leptin are independently associated with hip fracture .
