锁定加压钢板治疗老年股骨粗隆部粉碎性骨折60例疗效分析

目的探讨锁定加压钢板治疗老年股骨粗隆部粉碎性骨折的临床效果。方法对我院收治的60例老年股骨粗隆部粉碎性骨折患者随机分为对照组和观察组,观察组采用锁定加压钢板治疗,对照组采用动力髋螺钉内固定治疗,分析两组手术时间、术中出血量及术后患者恢复情况。结果观察组手术时间、术中出血量显著少于对照组,差异有统计学意义(P<0.05)。观察组与对照组优良率差异无统计学意义(P>0.05)。术后复查X线片提示所有患者均达解剖复位,未见前后成角或外翻,钢板位置良好未见断裂。所有患者均达骨性愈合,平均愈合时间(8.6±1.5)个月。结论采用锁定加压钢板治疗老年股骨粗隆部粉碎性骨折,优良率高,术后骨折愈合良好,并发症少,使用方便,值得临床推广应用。

Analysis of clinical effect of locking compression plate treated 60 cases of senile femoral trochanteric comminuted fracture

Objective To study the clinical effct of locking compression plate treated senile femoral trochanteric comminuted fracture. Methods Sixty cases of elderly femoral trochanteric comminuted fracture patients were randomly divided into control group and observation group, the observation group to be used of locking compression plate treatment, the control igroup to be used dynamic hip screw internal fixation treatment, analyzed of two groups of operation time, intraoperative amount of bleeding and postoperative recovery of patients, to be used locking compression plate treatment, analysis the operation time, intraoperative amount of bleeding and postoperative recovery of patients. Results The operation time, amount of bleeding during operation of observation group were significantly less than those of control group, and the difference was statistically significant （P 〈 0.05）. The excellent-good rates of observation group and control group had no significant difference （P 〉 0.05）. Postoperative review X ray film prompt all of the patients of anatomy reset, did not see the before and after into Angle or outer, steel plate position did not see good fracture. All of the patients of osseous healing, average healing time （8.6±1.5） months. Conclusion The use of locking compression plate treatment of senile femoral trochanter comminuted fracture, the excellent-good rate is high, postoperative fracture healing is good, fewer complications, easy to use, worth to application at clinical.
