药学服务下沉应用于慢性非传染性疾病效果分析

目的 探讨临床药师到社区开展药学服务(药学服务下沉)在慢性非传染性疾病中的应用效果,为规范医疗联合体内的药学服务提供参考。方法 通过问卷调查方式调查某市区2个基层医疗卫生机构的药学服务开展情况,针对薄弱环节进行专项培训、专项指导;对比药学服务下沉前后,医嘱审核拦截情况、用药指导例数、用药依从性、计划外就诊及再住院次数、药物治疗费用及满意度。结果 药学服务下沉后,共拦截不合理用药287例,提供用药指导732例;下沉后的依从性计划外就诊、再入院次数及药物治疗费用均显著减少,满意度评分显著升高(P<0.05)。结论 药学服务下沉有益于慢性非传染性疾病的控制,可规范医疗联合体内的药学服务。

Effect of Pharmaceutical Service Sinking in Chronic Non-Communicable Diseases

Objective To explore the value of the promotion and application of pharmaceutical service sinking in chronic non-communicable diseases,and to provide reference for regulating the pharmaceutical services in medical unions.Methods Questionnaire survey was conducted to obtain the situation of pharmaceutical services provided by 2 grassroots medical and health institutions in the city;special training and guidance for weak links was provided.The interception of doctor’s advice review,the number of cases of medication guidance,medication compliance and satisfaction,the number of unplanned visits and re-hospitalization,the cost of medical treatment and adverse reactions before and after the sinking of pharmaceutical services were compared.Results After the sinking of pharmaceutical services,a total of 287 cases of unreasonable drug use were examined and intercepted,and 732 cases of medication guidance were given.Compared with before sinking of pharmaceutical services,the compliance-associated unplanned visits,readmissions,and medication costs were significantly reduced,and satisfaction scores were significantly increased(P<0.05).Conclusion Pharmaceutical service sinking is beneficial to the control of non-communicable diseases,and can regulate the pharmaceutical services in the medical union.
