血清抗结核菌纯蛋白衍生物IgG测定对结核病的诊断意义

作者采用ELISA法检测血清抗结核菌纯蛋白衍生物(PPD)IgG抗体。结果表明,该试验诊断结核病的敏感性为93.4％,特异性为98.5％,预计值有效性为97.3％,对结核病,尤其是细菌学诊断尚有一定困难的肺外结核具有重要的辅助诊断价值。

Measurement of the serum level of IgG antibody to purified protein derivative by ELISA in the diagnosis of tuberculosis

An ELISA measuring serum IgG antibody to tuberculin purified protein derivative was evaluated. The results showed that the mean value of OD in tuberculosis group (n = 61 ) was significantly higher than that in healthy control subjects (n = 201, p<0.0l ) , and that in other nontuberculous diseases (n=147, p<0.0l). As the upper limit of normal range was set at 2 standard deviations above the mean for control values, the test had a sensitivity of 0.934, specificity of 0.985. The accuracy of positive prediction was 0.946, and the accuracy of negative prediction was 0.980. No difference could be found between the diagnostic sensitivity in patients with pulmonary tuberculosis and with extrapulmonary tuberculosis, except for tuberculous meningitis. So we suggested that this test might make a substantial contribution to the diagnosis of tuberculosis,especially to that of extrapulmonary tuberculosis in which the bacteriological diagnosis is often difficult.
