61例多囊卵巢综合征妇女行供精人工授精结局分析

目的探讨多囊卵巢综合征(polycystic ovary syndrome,PCOS)妇女采用不同治疗方案后行供精人工授精(artificial insemination by donor,AID)助孕的临床治疗效果。方法回顾性分析广东省计划生育专科医院61例妇女自然周期或是不同方案促排卵后行AID助孕122个周期,比较不同方案的临床妊娠率的差异。结果周期妊娠率20.83%,累积妊娠率40.98%,不同方案比较,妊娠率差异有统计学意义(P<0.05)。以克罗米芬(clomifene citrate,CC)/人绝经期促性腺激素(human menopausal gonadotropin,HMG)/人绒毛膜促性腺激素(human chorionic gonadotrophin,HCG)促排卵的妊娠率最高,卵泡刺激素(follicle stimulating hormone,FSH)/HCG组妊娠率与他莫昔芬(tamoxifen,TMX)/HMG/HCG组及CC/HMG/HCG组比较,差异有统计学意义(P<0.05)。结论 PCOS妇女调整周期自然排卵可以获得较高的妊娠率,CC/TMX与促性腺激素(gonadotropin,Gn)结合可以增加排卵率和临床妊娠率,减少Gn的应用时间,减少卵巢过度刺激综合征(Ovarian hyperstimulation syndrome,OHSS)的发生。

Outcomes analysis of artificial insemination by donor in 61 women with polycystic ovary syndrome

Objective To study clinical effect of artificial insemination by donor(AID) in 61 women with polycystic ovary syndrome(PCOS) by different treatments.Methods The data from 122 cycles AID by different methods stimulating ovulation of 61 women were retrospectively analyzed compared with clinical pregnancy rate in different stimulating methods.Results The pregnancy rate was 20.83% per cycle and the cumulative pregnancy rate was 40.98%.The different of pregnancy rate in different treatments was significant(P<0.05) that pregnancy rate of clomifene citrate(CC) /human menopausal gonadotropin(HMG) /human chorionic gonadotrophin(HCG) group was the highest.The difference of pregnancy rate was significant between follicle stimulating hormone(FSH) /HCG group and tamoxifen(TMX) /HMG /HCG group(P=0.042<0.05) or CC /HMG /HCG group(P=0.037<0.05).Conclusion The higher pregnancy rate of PCOS women can be gotten by regulation menstruation and natural ovulation.CC or TMX complicating gonadotropin(Gn) can get higher ovulation rate and clinical pregnancy rate and at the same time using time of Gn and the occurrence of ovarian hyperstimulation syndrome can reduce.
