卵巢癌患者血清人附睾分泌蛋白4和VEGF的表达水平及与预后的关系

目的 分析卵巢癌患者血清人附睾分泌蛋白4(HE4)和血管内皮生长因子(VEGF)表达水平,并探讨两者和患者预后的相关性.方法 在本院2010年9月至2013年12月实施治疗的卵巢癌患者中随机选取42例作为研究对象,为观察组;同时选取同期本院实施体检的健康女性42例作为对照组,采用酶联免疫吸附法检测两组研究对象HE4和VEGF表达水平,在观察组患者手术治疗后再次检测HE4和VEGF表达水平,对比两组的HE4和VEGF表达水平差异,对比观察组患者手术前后的HE4和VEGF表达水平变化,随访患者3～5年,调查分析患者预后和HE4、VEGF表达水平的关系.结果 观察组患者的HE4和VEGF表达水平和对照组相比,明显偏高(t=8.526、9.352,P＜0.05);手术后14 d观察组患者的HE4和VEGF表达水平和手术前相比,明显降低(t=10.253、9.574,P＜0.05);随访发现本次42例患者中复发5例,其中复发患者的HE4和VEGF表达水平均高于未复发患者,差异有统计学意义(P＜0.05);HE4和VEGF表达联合预测卵巢癌预后效果的ROC曲线下面积、准确性以及敏感度和两者单独预测相比,均偏高.结论 HE4和VEGF表达水平在卵巢癌诊断中具有重要应用价值,其中HE4和VEGF表达联合预测卵巢癌患者预后效果优于两者单独预测,因此说HE4联合VEGF在卵巢癌患者预后评估中具有重要应用价值.

The correlation in the expression levels of serum human epididymal secretory protein 4 and VEGF with prognosis in patients with ovarian cancer

Objective To analyze the correlation in the expression levels of serum human epididymal secretory protein 4 (HE4) and vascular endothelial growth factor (VEGF) with prognosis in patients with ovarian cancer.Methods The patients with ovarian cancer were randomly selected in 42 cases as a observation group in our hospital from September 2010 to December 2013,while 42 healthy women in health examination were selected as a control group in our hospital in the same time.The enzyme-linked immunosorbent assay was applied to detect the expression levels of HE4 and VEGF in the 2 groups.After operation treatment in the observation group,the expression levels of HE4 and VEGF were tested again.Comparing the expression levels of HE4 and VEGF between the 2 groups and between before and after operation in the observation group,we followed up the patients for 3-5 years to investigate and analyze the prognosis of patients with the expression levels of HE4 and VEGF.Results The expression levels of HE4 and VEGF were higher in the observation group than the control group (t =8.526,9.352,P ＜ 0.05).In 14 days after operation in the observation group,the expression levels of HE4 and VEGF were lower than that before operation (t =10.253,9.574,P ＜ 0.05).During the follow-up,there was tumor relapse in 5 cases,in which the expression levels of HE4 and VEGF were higher than those of patients without recurrence of tumor (P ＜ 0.05).Using combination of expression levels of HE4 and VEGF predicting the area under the receiver operating characteristic curve (ROC),accuracy and sensitivity was better than the expression levels of HE4 and VEGF alone.Conclusion The expression levels of HE4 and VEGF have important application value in diagnosis of ovarian cancer,of which combination of expression levels of HE4 and VEGF in predicting prognosis of patients with ovarian cancer are more satisfactory than their application separately,so that combined expression levels of HE4 with VEGF has important application value in predicting the prognosis in patients with ovarian cancer.
