改良B-Lynch缝合术联合欣母沛治疗宫缩乏力性产后出血的效果观察

目的 探究改良B-Lynch缝合术联合欣母沛治疗宫缩乏力性产后出血的效果.方法 选取2015年4月至2016年7月本院收治的剖宫产手术后宫缩乏力性产后出血患者90例,根据随机数表法分为两组,每组45例.对照组在常规止血的基础上实施欣母沛治疗,观察组在对照组的基础上实施改良B-Lynch缝合术治疗.记录两组患者产后2h、24 h出血量、产褥病发生率、并发症发生情况以及止血效果.结果 观察组产后2h出血量为(1 018.34±98.44) ml、24 h出血量为(503.42±26.98) ml,对照组产后2h出血量为(1 628.68±104.06) ml、24 h出血量为(769.02±28.22) ml,组间比较,差异均有统计学意义(均P＜ 0.05);观察组止血有效率为97.78％,明显高于对照组的82.22％,观察组产褥病发生率为6.67％,低于对照组的24.44％,差异均有统计学意义(均P＜0.05);观察组患者产后总并发症发生率为8.89％,与对照组的28.89％相比明显降低,差异有统计学意义(P＜0.05).结论 采用改良B-Lynch缝合术联合欣母沛治疗宫缩乏力性产后出血疗效显著,能有效减少出血量,降低产褥病和产后并发症发生率,止血较好,在临床应用中具有较高的推广价值.

Effect of modified B-Lynch suture combined with hemabate in treatment of postpartum hemorrhage caused by uterine inertia

Objective To investigate the effect of modified B-Lynch suture combined with hemabate in the treatment of postpartum hemorrhage caused by uterine inertia.Methods 90 patients with postpartum hemorrhage caused by uterine inertia treated at our hospital from April,2015 to July,2016 were selected and divided into a control group and an observation group with random number table,45 cases for each group.The control group took hemabate and routine hemostasis;in addition,the observation group took modified B-Lynch suture.The hemorrhage volumes 2 and 24 hours after delivery,the incidences of puerperalism and complications,and the hemostatic effect of these two groups were recorded.Results The hemorrhage volumes 2 and 24 hours after delivery were (1 018.34±98.44)ml and (503.42±26.98)ml in the observation group and were (1 628.68±104.06)ml and (769.02±28.22)ml in the control group,with statistical differences (both P＜0.05);The hemostatic effective rate was 97.78％ in the observation group and was 82.22％ in the control group.The incidence of puerperalism was lower in the observation group than in the control group (6.67％ vs.24.44％,P＜0.05).The incidence of postpartum complications was 8.89％ in the observation group and was 28.89％ in the control group (P＜0.05).Conclusions Modified B-Lynch suture combined with hemabate in the treatment of postpartum hemorrhage caused by uterine inertia has significant efficacy,can effectively reduce hemorrhage volume and decrease the incidences of puerperalism and postpartum complications,and has better hemostatic effect,so it is worth being clinically generalized.
