腹腔镜胆囊切除胆总管探查术的临床体会

目的:探讨腹腔镜联合胆道镜治疗胆囊结石合并胆总管结石的可行性生及安全性,总结手术方式的选择及术中注意事项.方法:回顾分析为42例胆囊结石合并胆总管结石患者行腹腔镜胆囊切除术、胆总管探查取石T管引流术的临床资料.结果:42例均顺利完成手术,手术时间75 ～185 min,平均130 min;术中出血量平均25 ml;结石取净率100％ 术后1例出现少许胆漏,无其他并发症发生,患者均顺利出院.结论:腹腔镜联合胆道镜治疗胆囊结石合并胆总管结石完全可行,手术安全、可靠,疗效良好.

The clinical experience of laparoscopic cholecystectomy and exploration of common bile duct

Objective:To explore the feasibility and security of cholecystolithiasis combined with common bile duct calculi management with laparoscopy and choledochoscopy,and to summarize the selection of the surgical procedures and precautions in the operation.Methods:The clinical data of 42 patients with cholecystolithiasis combined with common bile duct calculi who underwent laparoscopic cholecystectomy and common bile duct exploration and lithotomy and T-tube drainage were retrospectively analyzed.Results:All the 42 operations were accomplished successfully.The mean operative time was 130 min(ranged from 75 min to 185 min).The mean intraoperative blood loss was 25 ml.The stone free rate was 100%.Little bile leakage occurred in 1 case,no other complications occurred.All the 42 patients were successfully discharged from hospital.Conclusions:It is feasible,safe,reliable and effective to manage cholecystolithiasis combined with common bile duct calculi with laparoscopy and choledochoscopy.
