呼吸内科应用抗菌药物干预的疗效研究

目的探索实施合理使用抗菌药物干预的应用和效果。方法随机选取我院2011年1～8月入院的100例住院病例为干预组,2011年1月之前出院的100例为对照组,对干预组实施我院制订的合理用药干预方案进行干预。通过比较两组抗菌药物使用比例、抗菌药物联合用药情况、不合理使用抗菌药（用药无适应证、超级实用抗菌药、给药时机不当、用法用量错误、药物选择不当、用药时间过长）的发生情况、住院天数及抗菌药物费用所占比例等指标进行抗菌药物合理性应用评价。结果与对照组相比,干预组抗菌药物使用和抗菌药物联合用药比例、不合理使用抗菌药发生率以及平均抗菌药物费用占药费支出比例明显降低,住院时间明显缩短,差异均有统计学意义（P〈0.05）。结论实施长期持续全程干预措施对合理使用抗菌药是可行和有效的,符合卫生经济学高效节约的原则。笔者的经验有利于干预的拓展,为临床合理用药水平的提高奠定了基础。

Intervention study on the application of antimicrobial agents in department of respiratory

Objective To explore the application and effect of rational application of antimicrobial agents.Methods 100 patients who were discharged in hospital from January 2011 to August 2011 were chosen as intervention group.100 patients who were discharged randomly in hospital before January 2011 were chosen as control group.The intervention group was given the intervention plan of rational application of antimicrobial agents made by our hospital.The use of antibiotics,antimicrobial drug combination,the occurrence of irrational use of antimicrobial drugs(drug-free adaptation levy,super useful antibacterial drugs,improper time of administration,dosage errors,inappropriate choice of drugs,medication for too long),the average hospitalization days,and the cost of antibiotics in two groups were compared to evaluate the feasibility of application of antimicrobial agents.Results Compared with control group,the use of antibiotics,antimicrobial drug combination,the occurrence ratio of irrational use of antimicrobial drugs,the average hospitalization days and the cost of antibiotics in intervention group dropped(P<0.05).Conclusion The rational antimicrobial application in clinical practice is effective and practica1.Its efficiency savings are in line with the principles of health economics.Our experience will help to expand the intervention,and raise the level of the rational application of antimicrobial.
