体外静脉-静脉转流全身热疗治疗晚期癌症21例分析

摘    要:
目的探讨体外静脉-静脉转流灌注全身热疗(VV-PISH)对晚期肿瘤的疗效及安全性.方法 21例晚期恶性肿瘤患者在全麻状态下用TEMET 1000型体外循环全身热疗灌注治疗系统,进行全身加温治疗,通过临床观察及随访追踪,初步评估疗效.结果本疗法具有升温、降温平稳、控温简单、深部组织温度均匀的优点;具有明显止痛88.2%(15/17)作用,其杀伤抑制肿瘤有效率为57.1%(PR+SD),半年及1年病灶稳定率为63.2%(15/19)、46.7%(7/15),6个月、1年、1年6个月生存率分别为78.9%(15/19)、66.7%(10/15)、58.3%(7/12),部分患者甚至能达到消灭肿瘤的效果;虽然对重要器官组织(心、肺、肝、肾、脑)及血液有一定的副损伤,但其损伤是轻微和可逆的.结论 VV-PISH是一种安全有效的治疗方法,将为晚期恶性肿瘤病人的治疗带来新的希望.

Abstract:
Objective To investigate the effect and reliability of venovenous perfusion-induced systemic hyperthermia (VV-PISH) for advanced malignant tumor. Methods 21 patients with advanced malignant tumor had been treated by warmer therapy to whole body under general anesthesia, by clinical observation and by follow-up to evaluate the effect. Results This method has the following advantages, steadily increasing and decreasing temperature, easy manipulation, and isothermal in deep constitution. The curative effect were 88.2%(15/17) relieving pain, 57.1% inhibition tumor cell (PR+SD), 63.2% stationary phase of tumors in half year and 46.7% in one year. The 6 months, 1 year, 1year and 6 months survival rate are 78.9%(15/19), 66.7%(10/15) and 58.3%(7/12), respectively and especially, part of the patients'tumors were extinction. Although the method has some side-effects to the main organs (such as heart, lung, liver, brain and blood), but these damage is venial and reversible. Conclusion VV-PISH is a safe and effective method and will bring a new hope for the advanced malignant tumor patients.
